开展剪切波弹性成像指导下的神经源性呼吸衰竭膈肌功能评估与呼吸机撤离，为临床医生提供更加准确的诊断和治疗方案，提高患者的生活质量和治疗效果。  

Objectives of Study：

The diaphragm function assessment of neurogenic respiratory failure guided by shear wave elastography and ventilator withdrawal were carried out to provide clinicians with a more accurate diagnosis and treatment plan, and improve the quality of life and treatment effect of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄在18岁及以上，男女不限； 2.颅脑外伤、颅内感染、脑血管病、周围神经或肌肉病等神经源性疾病导致呼吸机撤离困难确诊为神经源性呼吸衰竭； 3.需要依赖呼吸机维持呼吸功能，持续使用呼吸机时间大于48小时； 4.无严重的心、肝、肾等器官功能不全，无严重感染、出血、肿瘤等并发症； 5.对超声检查或剪切力弹性成像无禁忌或过敏反应； 6.自愿参加本研究并签署知情同意书。

Inclusion criteria

1. The age of 18 years old and above, male or female; 2.Craniocerebral injury, intracranial infection, cerebrovascular disease, peripheral nerve or muscle disease and other neurogenic diseases leading to ventilator withdrawal difficulties were diagnosed as neurogenic respiratory failure; 3.Need to rely on ventilators to maintain respiratory function, continuous use of ventilators for more than 48 hours; 4.No serious heart, liver, kidney and other organ dysfunction, no serious infection, bleeding, tumor and other complications; 5.No contraindications or allergic reactions to ultrasound or shear-elastography; 6.Volunteer to participate in the study and sign the informed consent.

排除标准：

1.存在严重的肺部疾病或心血管疾病者； 2.具有其他原因如肺部疾病、胸廓畸形等引起的呼吸衰竭者； 3.存在膈肌或其他呼吸肌的结构或功能异常，如膈肌裂孔疝、肌营养不良等； 4.有影响剪切力弹性成像检查结果的因素，如腹水、气胸、胸腔积液等； 5.生命体征波动，有可能进一步恶化危及生命者。 6.在本研究期间发生严重的不良事件，如心跳骤停、呼吸停止、休克等； 7.在本研究期间主动退出或被研究者终止参与。

Exclusion criteria：

1.The patients with serious lung disease or cardiovascular disease; 2.Respiratory failure caused by other causes such as lung disease, thoracic deformity, etc.; 3.There are structural or functional abnormalities of the diaphragm or other respiratory muscles, such as diaphragmatic hiatus hernia, muscular dystrophy, etc.; 4.There are factors that affect the results of shear elastography, such as ascites, pneumothorax, pleural effusion, etc; 5.Vital signs fluctuate, may be further aggravated life-threatening; 6.Serious adverse events occurred during the study period, such as cardiac arrest, respiratory arrest, shock, etc. 7.During the study period, the participant voluntarily withdrew or was terminated by the researcher.
