研究目的：

回顾分析本中心2023年1月1日-2026年6月30日经T-ESD及C-ESD治疗的早期胃癌及癌前病变病例，对比剥离时间、剥离速度、肌层损伤、穿孔等关键指标，以期评价AMLT装置的有效性和安全性。  

Objectives of Study：

Retrospective analysis of early gastric cancer and precancerous lesions treated with T-ESD and C-ESD in our center from January 1, 2023 to December 31, 2025, comparing key indicators such as dissection time, dissection speed, muscle layer damage, perforation, etc., in order to evaluate the effectiveness and safety of AMLT devices.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄18岁以上； （2）通过放大内镜或染色内镜诊断为早期胃癌及癌前病变，肿瘤位于胃底、胃体或胃角，且直径≥20mm； （3）肿瘤符合日本胃癌治疗规约（第二版）的绝对适应症和扩大适应症: a.粘膜内分化型腺癌，无溃疡；b.合并溃疡的粘膜内分化型腺癌，≤30mm； （4）术前病理活检诊断符合胃癌Group分型3、4 and 5； （5）患者活动状态ECOG评分0–2级

Inclusion criteria

(1) Age 18 and above; (2) Early gastric cancer and precancerous lesion is diagnosed through magnifying endoscopy or staining endoscopy, and the tumor is located at the gastric fundus, gastric body, or gastric angle with a diameter of >= 20mm; (3) The tumor meets the absolute and expanded indications of the Japanese Gastric Cancer Treatment Protocol (Second Edition): a. Differentiated adenocarcinoma within the mucosal layer, without ulcers; b. Differentiated adenocarcinoma within the mucosal layer of combined ulcers, <= 30mm; (4) Preoperative pathological biopsy diagnosis is consistent with gastric cancer group subtypes 3, 4, and 5; (5) Patient activity status ECOG score 0-2.

排除标准：

（1）术前诊断浸润深度达黏膜下深层（≥SM2）侵犯的早期胃癌患者； （2）经腹部增强CT扫描发现有淋巴结转移的转移； （3）内镜切除术后局部复发的病例； （4）合并胃外活动性恶性肿瘤的患者； （5）严重贫血、凝血功能障碍、心肺肝肾功能不全、严重急性感染、妊娠期患者； （6）不能耐受静脉麻醉病例； （7）不能停用抗凝血剂和抗凝血剂（低剂量阿司匹林除外）； （8）正在参与其他临床研究的患者。

Exclusion criteria：

(1) Preoperative diagnosis of early gastric cancer patients with invasion depth reaching the submucosal deep layer (>= SM2); (2) Enhanced CT scan of the abdomen revealed lymph node metastasis; (3) Cases of local recurrence after endoscopic resection; (4) Patients with active malignant tumors outside the stomach; (5) Severe anemia, coagulation dysfunction, heart, lung, liver, and kidney dysfunction, severe acute infection, and pregnant patients; (6) Unable to tolerate intravenous anesthesia cases; (7) Anticoagulants and anticoagulants (excluding low-dose aspirin) cannot be discontinued; (8) Patients who are currently participating in other clinical studies.
