研究目的：

对研究地区中小学生的主要伤害问题及其影响因素进行的流行病学诊断、行为与环境诊断、教育与生态学诊断、管理与政策诊断，为构建适用于北京地区的基于家庭学校社区多维度框架中小学生伤害综合干预模式和干预措施提供依据。  

Objectives of Study：

The epidemiological diagnosis, behavioral and environmental diagnosis, educational and ecological diagnosis, and management and policy diagnosis of the main injury problems and their influencing factors among primary and secondary school students in the study area provide a basis for constructing a comprehensive intervention model and measures for student injuries based on a multidimensional framework of family, school, and community, which is applicable to the Beijing area.

纳入标准：

1. 学生调查对象 纳入标准为：（1）学生：小学4-5年级学生，初中1-2年级学生，高中1-2年级学生；（2）过去12个月内在调查地区范围内居住时间累计不低于6个月。（3）能进行有效交流；（4）签署知情同意书，愿意参加本研究。 2. 项目相关单位调查对象 调查地区卫健、疾控、教育、公安、交通、消防、妇联等学生安全工作相关部门工作人员； 调查地区抽样学校所在街道办事处/乡镇政府工作人员； 调查地区抽样学校教师和工作人员。

Inclusion criteria

Inclusion Criteria for student: 1.Students in grades 4–5 of primary school, grades 1–2 of junior high school, and grades 1–2 of senior high school. 2.Have resided locally for no less than six months in last 12 months. 3.Capable of effective communication. 4.Signed informed consent form and willingness to participate in this study. Inclusion Criteria for investigation subjects related to the project 1. Staff members from relevant departments of student safety work in the investigation area, such as health and family planning, disease control, education, public security, transportation, fire control, and women's federations. 2. Staff members of the sub-district offices/township governments where the sampled schools are located in the investigation area. 3. Teachers and staff of the sampled schools in the investigation area.

排除标准：1）处于疾病急性期或慢性病急性发作期；2）自身存在认知或交流障碍。

Exclusion Criteria: 1.In the acute phase of an illness or experiencing an acute exacerbation of a chronic disease. 2.Presence of cognitive or communication impairments.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集 1. 学生的问卷调查由调查学校组织学生在校内集中以自填方式完成。项目工作人员将问卷统一发放至学校，并对学校负责调查老师进行调查培训，负责调查老师统一向学生发放调查问卷。现场调查实施前，需完成学生的知情同意过程。其中，高中生需本人签署知情同意书，小学生和初中生需要本人和家长均签署知情同意书。现场调查时，负责调查老师/工作人员需按照项目要求，统一向学生说明问卷填写方法并对核心概念进行解释说明，学生现场填写问卷，老师统一收集问卷。 2. 项目相关单位调查由疾控中心负责将调查表发至相关单位，并确定调查单位名单。项目相关单位相关负责人员自填完成调查表后，由疾控中心人员负责统一收集调查表。 数据管理 1. 数据将通过Epidata进行录入，设置录入逻辑校检规则，由专人、使用专门的电脑录入，电脑设有密码。双人双录入。以Excel表格形式保存。数据分析前将进行脱敏处理，去掉个人信息。 2. 将对数据的完整性、准确性、一致性等方面进行核查。 3. 数据备份将备份在一台无接触互联网的电脑上，确保数据的安全性和可恢复性。 4. 数据将进行加密，只有核心研究成员拥有数据访问权限，以保护数据的隐私和保密性。由专人负责对数据进行清洗分析，分析时注意保密原则。 5. 发布、使用信息时仅使用与健康相关信息，不涉及个人姓名、图像等隐私信息。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data Collection 1. The student questionnaire survey is organized by the schools, with students completing it through self-administration. Project staff will distribute the questionnaires uniformly to the schools and provide survey training to the teachers in charge of the survey at the schools. These teachers will then distribute the questionnaires to the students. Before the on-site survey is implemented, the informed consent process for the students must be completed. Specifically, high school students need to sign the informed consent form themselves, while primary and middle school students need both themselves and their parents to sign the informed consent form. During the survey, the teachers/staff must, in accordance with project requirements, uniformly explain to the students the method of filling out the questionnaire and provide explanations for core concepts. The students will fill out the questionnaire on-site, and the teachers will collect the questionnaires uniformly. 2. The investigation of project-related units is carried out by the Centers for Disease Control and Prevention (CDC), which is responsible for distributing the survey forms to the relevant units and determining the list of surveyed units. After the relevant personnel from the project-related units complete the survey forms through self-administration, the CDC staff will be responsible for uniformly collecting the survey forms. Data Management 1. Data will be entered through Epidata, with logical validation rules set for data entry. A dedicated person will use a specific computer to enter the data, and the computer will be password-protected. Double data entry will be conducted. The data will be saved in the form of an Excel spreadsheet. Before data analysis, data will be anonymized by removing personal information. 2. The integrity, accuracy, and consistency of the data will be verified. 3. Data backups will be stored on a computer that is not connected to the Internet, ensuring the security and recoverability of the data. 4. Data will be encrypted, and only core research members will have access to the data to protect data privacy and confidentiality. A designated person will be responsible for data cleaning and analysis, with attention to confidentiality principles during the analysis. 5. When publishing or using information, only health-related information will be used, without involving personal names, images, or other private information.
