研究目的：

本研究以 Snyder 希望理论模型为理论框架，有计划地为老年高血压患者设计规范化、个性化、科学化、合理化的护理干预方案。以福建省某所三级医院心血管内科的老年高血压患者作为研究对象，对照组实施常规护理干预方案，研究组在实施常规护理的基础上实施基于 Snyder 希望理论的护理干预方案，并在干预结束后开展3个月的随访期，比较老年高血压患者希望水平、高血压患者主动健康行为得分、生活质量SF-12量表得分、急性并发症发生情况及血压有无差异，旨在探讨基于 Snyder 希望理论的护理干预对老年高血压患者的影响，为促进老年高血压患者的主动健康与疾病预后提供参考方案。  

Objectives of Study：

In this study, Snyder's hope model was used as the theoretical framework to design a standardized, personalized, scientific and rational nursing intervention plan for elderly patients with hypertension. Elderly patients with hypertension in the cardiovascular department of a tertiary hospital in Fujian Province were taken as the study objects. The control group was implemented with routine nursing intervention program, while the research group was implemented with nursing intervention program based on Snyder's hope theory on the basis of routine nursing, and a 3-month follow-up period was carried out after the intervention. Compare hope level, active health behavior score, quality of life SF-12 scale score, occurrence of acute complications and blood pressure of elderly hypertensive patients to explore the impact of nursing intervention based on Snyder hope theory on elderly hypertensive patients. To provide a reference scheme for promoting active health and disease prognosis in elderly patients with hypertension.

纳入标准：

1.高血压符合《中国高血压防治指南2018年修订版》中的标准； （2）非高龄老年高血压：65岁≤年龄≤80岁； （3）近 6 个月内没有遭受过其他重大创伤性事件，如丧偶、车祸等； （4）自愿参加本研究并签署知情同意书者； （5）能够阅读理解问卷内容，独立或在研究者帮助下完成问卷。

Inclusion criteria

1. Hypertension meets the criteria in the 2018 Revised Chinese Guidelines for Hypertension Prevention and Treatment ; 2. Non-elderly hypertension: 65 years<=age <=80 years ; 3. have not suffered other major traumatic events in the last 6 months, such as the death of a spouse, a car accident, etc. 4. Volunteer to participate in the study and sign the informed consent; 5. Be able to read and understand the contents of the questionnaire and complete the questionnaire independently or with the help of the researcher.

排除标准：

1.继发性高血压、急进性高血压的患者； （2）肝、肾、脑等严重器官疾病或肿瘤者； （3）精神、人格和意识障碍，不能正确回答问题者； （4）病人存在听力、视觉障碍； （5）病人抑郁存在自伤、伤人等精神疾病，或精神疾病家族史者。

Exclusion criteria：

1. Patients with secondary hypertension and acute hypertension; 2. liver, kidney, brain and other serious organ diseases or tumors; 3. mental, personality and consciousness disorders, unable to answer questions correctly; 4. The patient has hearing and visual impairment; 5. Patients with depression have self-injury, wounding and other mental diseases, or family history of mental illness.
