研究目的:

脑卒中事件具有发病率高、致残率高及死亡率高的特点,动脉粥样硬化是缺血性脑卒中的最常见病因,尽管多数研究证实脑卒中发生率与血管狭窄程度呈正相关,但轻度管腔狭窄的病人脑卒中发生率也在上升,因此研究颈内动脉易损斑块的成分尤为重要。无创性斑块成像技术飞速发展,对颈动脉粥样硬化的诊疗提供了非常有利的手段,能够精准快速地识别易损斑块,人工智能在CT、MRI及超声对颈动脉易损斑块特征已有很多研究,但是对颈动脉斑块进行风险分层方面,尚未有研究将能谱 CT及MRI定量参数整合入机器学习模型中,临床未广泛使用,仍需要进行大规模的前瞻性纵向研究。  

Objectives of Study:

Stroke events are characterized by high incidence, high disability rate, and high mortality rate. Atherosclerosis is the most common cause of ischemic stroke. Although most studies have confirmed a positive correlation between the incidence of stroke and the degree of vascular stenosis, the incidence of stroke is also increasing in patients with mild luminal stenosis. Therefore, it is particularly important to study the components of vulnerable plaques in the internal carotid artery. Non-invasive plaque imaging technology has developed rapidly, providing very favorable means for the diagnosis and treatment of carotid atherosclerosis. It can accurately and quickly identify vulnerable plaques. There have been many studies on the characteristics of carotid vulnerable plaques by artificial intelligence in CT, MRI, and ultrasound. However, in terms of risk stratification of carotid plaques, no study has integrated the quantitative parameters of spectral CT and MRI into machine learning models yet. It has not been widely used in clinical practice and still requires large-scale prospective longitudinal studies.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1、超声检查提示颈动脉低回声斑块(即软斑块)并斑块厚度>1.5mm,对患者进行基本情况告知并同意进一步进行颈部CTA及MRA检查 2、年龄40~80岁; 3、近期未发生过脑血管不良事件。

Inclusion criteria

1. Ultrasound examination showed carotid hypoechoic plaque (i.e., soft plaque) and plaque thickness > 1.5mm, and the patient was informed of the basic situation and agreed to further neck CTA and MRA examination 2. Age 40~80 years old; 3. There have been no cerebrovascular adverse events recently.

排除标准:

1、对影像学检查存在禁忌症者; 2、合并严重感染者; 3、有精神疾病无法配合观察者; 4、图像质量差,存在运动伪影或难以绘制感兴趣区的; 5、同时参加其他研究者。 6、不同意参加本研究者

Exclusion criteria:

1. Those who have contraindications to imaging examination; 2. Patients with severe infection; 3. Mentally ill and unable to cooperate with the observer; 4. Poor image quality, motion artifacts or difficulty in drawing the region of interest; 5. Participate in other researchers at the same time. 6. Those who do not agree to participate in this study
