研究目的：

1.主要目的 评估MPFL重建术联合骨软骨骨折固定术的中长期疗效（功能恢复、复发率、影像学改变）。 比较MPFL修复术联合骨软骨骨折固定术的远期复发率，及骨软骨损伤愈合情况，以及对预后的影响。 2.次要目的 分析术后并发症发生率及危险因素。 探讨患者生活质量（运动能力、日常活动）与手术效果的关系。  

Objectives of Study：

1. Main purpose To evaluate the medium and long term efficacy of MPFL reconstruction combined with fixation of osteochondral fractures (functional recovery, recurrence rate, imaging changes). To compare the long-term recurrence rate, the healing of osteochondral injury and the effect on prognosis of MPFL repair combined with fixation of osteochondral fracture. 2. Secondary purpose The incidence of postoperative complications and risk factors were analyzed. To explore the relationship between patients' quality of life (exercise ability, daily activities) and surgical effect.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄10-18岁，首次或复发性髌骨脱位合并骨软骨骨折（经MRI或关节镜确诊）。 骨折块直径≥10mm或存在游离体需手术固定。 接受MPFL重建术或修复术联合骨折内固定/微骨折处理。 保守治疗失败（如复发性脱位、持续疼痛）。

Inclusion criteria

1.Age 10-18 years, first or recurrent patellar dislocation with osteochondral fracture (confirmed by MRI or arthroscopy); 2.Fracture mass diameter >=10mm or the presence of free body requires surgical fixation; 3.Underwent MPFL reconstruction or prosthesis combined with internal fixation/microfracture management; 4.Failure of conservative treatment (e.g. recurrent dislocation, persistent pain).

排除标准：

合并其他膝关节韧带损伤（如ACL断裂）。 既往有髌骨手术史或严重骨关节炎（Outerbridge分级≥III级）。 合并发育性髌股关节畸形（如高位髌骨、滑车发育不良），行矫正手术。 无法完成术后随访（如异地居住、精神疾病）。

Exclusion criteria：

1.Combined with other knee ligament injuries (such as ACL rupture); 2.Prior patellar surgery or severe osteoarthritis (Outerbridge grade >=III); 3.Combined with developmental patellofemoral malformations (such as high patellar and trochlear dysplasia), corrective surgery was performed; 4.Inability to complete post-operative follow-up (e.g. remote living, mental illness).
