旨在基于二元疾病管理理论的指导完成慢性心衰共病患者家庭参与式智能管理支持系统的构建与验证，以实现二元整体居家管理期间症状动态变化的实时监测，并针对性地提供个性化疾病管理“套餐”，为二元整体出院后健康管理提供精准支持，提高管理效率和管理质量，促进二元整体出院后健康发展。  

Objectives of Study：

The aim is to construct and validate a participatory intelligent management support system for families of chronic heart failure comorbidities based on the guidance of binary disease management theory, in order to achieve real-time monitoring of dynamic changes in symptoms during binary overall home management, and provide personalized disease management "packages" targeted at providing precise support for binary overall health management after discharge, improving management efficiency and quality, and promoting the healthy development of binary overall discharge.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者：①入院时符合《中国心力衰竭诊断和治疗指南2018》中心衰患者诊断标准；②以心衰为主要入院诊断；③年龄≥18岁；④有阅读和语言表达能力，沟通无障碍；⑤在研究期间使用智能手机。 照顾者： ①每天照护时间超过6小时；②患者家属或近亲属；③有阅读和语言表达能力，沟通无障碍；④在研究期间使用智能手机。

Inclusion criteria

Patients: 1. Meeting the diagnostic criteria for heart failure patients in the 2018 Chinese Guidelines for the Diagnosis and Treatment of Heart Failure upon admission; 2. Cardiac failure as the primary admission diagnosis; 3. Age >= 18 years old; 4. Ability to read and express oneself in language, with no communication barriers. Caregiver: 1. Care time exceeding 6 hours per day; 2. Family members or close relatives of the patient; 3. Ability to read and express oneself in language, with no communication barriers; 4. Use smartphones during the research period.

排除标准：

患者：①近3 个月发生过心肌梗死、中风、慢性阻塞性肺疾病、肾脏衰竭、癌症等（合并上述疾病对心衰患者症状产生复合效应）；②出院后拟进入照护机构；③近3 个月参加过常规护理之外的其他干预试验。 照顾者：①有偿照顾者；②近3个月参加过常规管理之外的其他干预试验。

Exclusion criteria：

Patients:1. Having experienced myocardial infarction, stroke, chronic obstructive pulmonary disease, renal failure, cancer, etc. in the past 3 months (combined with the above diseases, the symptoms of heart failure patients have a compound effect); 2. Intending to enter a care facility after discharge; 3. having participated in intervention trials other than routine care in the past three months. Caregivers: 1. Paid caregivers; 2. participated in intervention trials other than routine management in the past three months.
