研究目的：

探讨超声辅助定位改良椎旁入路与触摸解剖定位传统椎旁入路在蛛网膜下腔麻醉中的操作时间、穿刺成功率及患者满意度的差异。 验证改良椎旁入路技术在减少麻醉穿刺相关并发症（如头痛、腰痛等）方面的潜在优势。 确定超声辅助技术是否能提供更加安全、有效的椎管内麻醉方案，尤其是针对老年髋部骨折患者。  

Objectives of Study：

To evaluate the clinical efficacy of the ultrasound-assisted modified paramedian approach in spinal anesthesia for elderly patients with hip fractures. To compare the success rate, number of punctures, procedure time, and patient satisfaction between the modified paramedian approach and the traditional landmark-based method. To investigate the reduction in complications associated with spinal anesthesia using ultrasound guidance.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 髋部骨折需在蛛网膜下腔麻醉下手术 2: 性别不限 3: 美国麻醉医师协会（ASA）分级为I~III级 4: BMI 18-35 5. 年龄>=65岁

Inclusion criteria

1.The hip fracture surgery should be performed under subarachnoid block anesthesia; 2.Gender: No restriction;; 3.American Society of Anesthesiologists (ASA) physical status classification I-III; 4.BMI 18-35; 5.Age>=65 years old.

排除标准：

1: 穿刺部位感染 2: 休克患者 3: 凝血功能障碍 4: 颅内压增高 5: 腰椎手术史 6: 脊柱侧弯 7: 妊娠期、哺乳期妇女

Exclusion criteria：

1.Infection at the puncture site; 2.Patients with shock; 3.Coagulation dysfunction; 4.Increased intracranial pressure; 5.History of lumbar spine surgery; 6.Scoliosis; 7.Pregnant or breastfeeding women.
