研究目的:

1.本项目依据心衰不同分级对心力衰竭患者的肠道菌群特征性结构、基因功能变化的进行阐释;揭示特定菌群与特定代谢物(群)之间的关联性。多角度建立从微生物水平及机体代谢水平上的心力衰竭与肠道菌群两者之间相互作用机制。将肠道菌群作为新的切入点,深入研究心力衰竭的发生和发展分子机制,可为心血管疾病的预防和治疗提供新的思路; 2. 对TMA-TMAO、SCFAs和Bile acids三条代谢通路中的差异代谢物进行靶向绝对定量测定。揭示特定菌群与特定代谢物(群)之间的关联性。建立心力衰竭与肠道菌群相互作用的代谢通路网络图。探索心力衰竭与肠道菌群之间的代谢途径和作用机制是亟需突破的研究重点及难点; 3. 整合多种机器学习LR、RF以及SVM算法,建立具有临床预测心力衰竭发生发展的功能模型,以期为临床上心力衰竭的发生发展提供指导。为临床上使用预防心血管疾病事件发生提供新线索;为改善和提高临床药物个性化用药应用提供理论基础。  

Objectives of Study:

1. This project elucidates the structural and functional changes in gut microbiota among patients with heart failure across different severity grades, revealing associations between specific bacterial communities and their corresponding metabolite groups. By integrating microbial and metabolic perspectives, it establishes the interaction mechanisms linking heart failure with gut microbiota at both microbial and systemic metabolic levels. Leveraging gut microbiota as a novel entry point for further investigation into the molecular mechanisms underlying the progression of heart failure can offer innovative strategies for the prevention and treatment of cardiovascular diseases; 2. Differential metabolites within three key metabolic pathways—TMA-TMAO, SCFAs, and bile acids—are quantified using absolute targeted analysis. These findings uncover correlations between specific bacterial communities and their associated metabolite groups, enabling the construction of an interactive metabolic pathway network map that delineates the relationship between heart failure and gut microbiota. Investigating these metabolic pathways and mechanisms represents a critical research focus and challenge in the field; 3. A range of machine learning algorithms, including logistic regression (LR), random forest (RF), and support vector machine (SVM), are integrated to develop a predictive model for assessing the onset and progression of heart failure in clinical settings. This approach not only aids in understanding the trajectory of heart failure but also provides new insights into preventing cardiovascular events and enhances the theoretical foundation for advancing personalized medicine in clinical practice.

纳入标准:

病例选择: 在陕西省人民医院门诊及心内科诊断为心力衰竭患者约120例,按心衰患者的分级不同(NYHA IV级、NYHA Ⅲ级、NYHA Ⅱ级、NYHA Ⅰ级),每组30例,年龄在55-75岁之间。心力衰竭的诊断依据:2021年ESC欧洲急慢性心力衰竭诊断与治疗指南。 健康对照: 在社区选取健康者30例,年龄均在55-75岁之间的健康人群。

Inclusion criteria

Case selection: About 120 patients with heart failure were diagnosed in the outpatient and cardiology departments of Shaanxi Provincial People's Hospital, and there were 30 patients in each group, aged between 55 and 75 years, according to the classification of heart failure patients (NYHA class IV, NYHA class III., NYHA class II., and NYHA class I). Diagnosis of heart failure is based on: ESC European guidelines for the diagnosis and treatment of acute and chronic heart failure, 2021. Healthy Controls: A total of 30 healthy people aged 55-75 years were selected in the community.

排除标准:

病例选择: 1.三个月内有抗生素应用史; 2.长期服用类固醇激素、避孕药、甲状腺激素; 3.糖尿病、肥胖、肝肾疾病、脑血管疾病、风湿结缔组织疾病; 4.甲状腺功能低下; 5.肾上腺皮质功能亢进; 6.急慢性胃肠道疾病; 7.其他心血管疾病用药; 8.中途拒绝参与的患者。 健康对照: 高血压病、冠心病、糖尿病等心血管疾病史及患有胃肠道疾病或进行过胃肠手术者。

Exclusion criteria:

Case selection: 1. History of antibiotic use within three months; 2. Long-term use of steroid hormones, contraceptives, and thyroid hormones; 3. Diabetes, obesity, liver and kidney diseases, cerebrovascular diseases, rheumatic connective tissue diseases; 4. Hypothyroidism; 5. Adrenal hyperfunction; 6. Acute and chronic gastrointestinal diseases; 7. Other drugs for cardiovascular diseases; 8. Patients who refuse to participate halfway. Healthy Controls: History of cardiovascular diseases such as hypertension, coronary heart disease, diabetes, and those with gastrointestinal diseases or gastrointestinal surgery.
