提出我国老年糖前患者血中 25(OH)D适宜界值，为T2D的一级预防提供科学证据。  

Objectives of Study：

To provide scientific evidence for the primary prevention of T2D by proposing the appropriate cut-off value of serum 25(OH)D in elderly prediabetic patients.

纳入标准：

第一部分纳入标准： 2022-2024 年连续 3 年参与健康体检，且有完整的相关信息（年龄、腰围、BMI、Hb、教育程度、吸烟、饮酒情况）、血脂指标（血清总胆固醇、HDL，LDL、TG）、血糖指标（空腹血糖）、血压指标（SBP、DBP）的老年人中，通过 FPG 和 HbA1c 检测，采用目前国际通用的 ADA（2025 年）标准进行糖尿病前期的诊断： ①空腹血糖受损（IFG）：5.6<=FPG<7.0mmol/L； ②糖化血红蛋白（HbA1c）:5.7%<=HbA1c＜6.5% （满足任意一项）。 第二部分纳入标准： ①年龄 65-80 岁，BMI 20-26.9kg/m^2，一般状况良好，生活自理； ②血清 25（OH）D 水平<12ng/ml； ③结合 OGTT 试验，采用美国糖尿病协会（ADA）2025 年标准进行糖尿病前期的诊断（符合其中一项即可诊断为 pre-DM）： a. 空腹血糖受损（IFG）：5.6<=FPG<7.0 mmol/L; b. 糖耐量异常（IGT）：FPG<7.0 mmol/L；7.8<=PP2h<11.0 mmol/L; c. 糖化血红蛋白异常：5.7%<=HbA1c＜6.5% * 注：IFG 和 IGT 统称为糖调节受损（IGR）

Inclusion criteria

Inclusion criteria for the first part: From 2022 to 2024, among the elderly who participated in health examinations for three consecutive years and have complete relevant information (age, waist circumference, BMI, Hb, education level, smoking, drinking status), lipid indicators (serum total cholesterol, HDL, LDL, TG), blood glucose indicators (fasting blood glucose), and blood pressure indicators (SBP, DBP), the diagnosis of prediabetes was made using FPG and HbA1c tests according to the currently internationally accepted ADA (2025) standards: (1) Impaired fasting glucose (IFG): 5.6 <= FPG < 7.0 mmol/L; (2) Glycated hemoglobin (HbA1c): 5.7% <= HbA1c < 6.5% (meeting either criterion). Inclusion criteria for the second part: (1) Ages 65-80, BMI 20-26.9 kg/m^2, in generally good condition, able to live independently; (2) Serum 25(OH)D level < 12 ng/ml; (3) Combined with OGTT tests, using the American Diabetes Association (ADA) 2025 standards for diagnosing prediabetes (meeting any one criterion can be diagnosed as pre-DM): a. Impaired fasting glucose (IFG): 5.6 <= FPG < 7.0 mmol/L; b. Abnormal glucose tolerance (IGT): FPG < 7.0 mmol/L; 7.8 <= PP2h < 11.0 mmol/L; c. Abnormal glycated hemoglobin: 5.7% <= HbA1c < 6.5%. * Note: IFG and IGT are collectively referred to as impaired glucose regulation (IGR).

排除标准：

1.第一部分排除标准：排除已诊断为 T2D 的人群 2.第二部分排除标准： （1）新诊断或自我报告的糖尿病(FPG>=7.0 mmol/L 和/或 2 h-PG>=11.1 mmol/L 和/或正在进行的降糖治疗)； （2）过去一年有严重症状的心血管疾病史（如不稳定型心绞痛、心肌梗死或中风）； （3）肝肾功能受损； （4）骨改变情况（如甲状腺功能亢进、肾结石和结节病）； （5）高钙血症。

Exclusion criteria：

1. Exclusion criteria for the first part: Exclude populations diagnosed with T2D. 2. Exclusion criteria for the second part: (1) Newly diagnosed or self-reported diabetes (FPG >= 7.0 mmol/L and/or 2h-PG >= 11.1 mmol/L and/or ongoing hypoglycemic treatment); (2) A history of severe symptoms of cardiovascular disease in the past year (such as unstable angina, myocardial infarction, or stroke); (3) Impaired liver or kidney function; (4) Bone changes (such as hyperthyroidism, kidney stones, and granulomatous disease); (5) Hypercalcemia.
