研究目的：

探讨在GDM患者中，剖宫产术前低碳水化合物饮水是否能显著降低新生儿低血糖风险(优效性界值≥20%），同时不导致孕妇围手术期血糖的异常升高(≥1mmol/L）， 不增加母儿血糖相关的围手术期并发症。  

Objectives of Study：

To determine whether low-carbohydrate watering before caesarean section is significantly superior in reducing the risk of neonatal hypoglycaemia (>=20%) in patients with GDM, without leading to abnormal maternal glycaemia (>=1 mmol/L) and without increasing perioperative glycaemia-related complications.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.拟单中心招募在中山大学附属第三医院行计划性剖宫产术、血糖控制良好的GDM孕妇。 纳入标准：单胎、本孕期诊断为GDM、孕期经饮食控制或使用降糖药物后血糖控制良好： 餐前：3.3-5.3mmol/L；餐后2小时4.4-6.7mmol/L，HbA1c＜5.5%，孕龄≥37周，并且接受计划 剖宫产、采用椎管内麻醉方式。

Inclusion criteria

1. We plan to recruit GDM pregnant women who undergo planned cesarean section and have good blood glucose control at the Third Affiliated Hospital of Sun Yat sen University for a single center recruitment. Inclusion criteria: singleton, diagnosed with GDM during this pregnancy, good blood glucose control after dietary control or use of hypoglycemic drugs during pregnancy: pre meal: 3.3-5.3mmol/L; 4.4-6.7 mmol/L 2 hours after meals, HbA1c<5.5%, gestational age >= 37 weeks, and undergoing planned cesarean section and spinal anesthesia.

排除标准：

1.紧急剖宫产，接受全身麻醉，既往有胃排空障碍，合并严重的心肺功能异常、 严重高血压（BP≥160/110mmHg），自身免疫性疾病、合并活动性感染，B级及以上的GDM，孕 前糖尿病，完全性前置胎盘，患有精神心理疾病等不能良好配合饮食，合并严重胎儿畸形。

Exclusion criteria：

1. Emergency cesarean section, general anesthesia, previous gastric emptying disorder, severe cardiopulmonary dysfunction, severe hypertension (BP >= 160/110mmHg), autoimmune disease, active infection, GDM of grade B and above, pre pregnancy diabetes, complete placenta previa, psychosocial disease, and severe fetal malformation.

由非参与研究的统计学专家生成随机序列，具体如下：按入院时间连续8个病人为一个区组，每个区组均按1:1比例 将入组患者分配至碳水组及常规组，每个区组在首例患者入组前，使用 Randomization.com ，通过计算机生成序列号并随机选择4个序列号为ERAS组（相应卡片 标识为A），剩下的分配到对照组(相应卡片标识为B)。

In a block randomization design, every eight consecutive patients was treated as a single block based on the time of admission, and patients within a block group were randomly assigned to the low-carbohydrate watering group and the conventional group in a 1:1 ratio.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

患者招募由本课题组2名3年以上主治医师进行筛选和入组，每例入组患者的相关数据均使用CRF表记录，并录入EXCEL表进行互相核查。
