研究目的：

本研究旨在构建并验证数字化高血压患者自我管理干预模式，以提升患者的健康素养和自我管理能力，优化疾病管理效果。  

Objectives of Study：

The study aims to develop and validate a digital self-management intervention for patients with hypertension, with the goal of enhancing their health literacy and self-management capabilities, thereby improving the effectiveness of disease management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）既往被医疗机构诊断为原发性高血压且已开始服用降压药的患者。（2）年龄≥18岁，研究对象为成年高血压患者。 （3）线上干预需具备读写能力，可以使用智能手机和微信客户端，以便更好地参与干预。（4）知情同意，自愿参加研究。

Inclusion criteria

(1) Patients previously diagnosed with primary hypertension by a medical institution and currently taking antihypertensive medications. (2) Aged 18 years or older; participants must be adults with hypertension. (3) Possess basic literacy skills and the ability to use a smartphone and the WeChat application to facilitate participation in the online intervention. (4)Provide informed consent and voluntarily agree to participate in the study.

排除标准：

1）未开始服用降压药的患者（2）有心脑血管疾病、肌肉、关节疼痛等严重并发症等运动禁忌症，或者医生建议不能参加运动的 （3）具有精神障碍的患者，无法自主参与健康干预项目

Exclusion criteria：

1) Patients who have not started taking antihypertensive drugs 2) Those with severe complications such as cardiovascular and cerebrovascular diseases, muscle and joint pain, or other contraindications to exercise, or those advised by doctors not to participate in exercise 3) Patients with mental disorders who are unable to independently participate in health intervention programs
