研究目的：

通过本研究，评估高压氧舱内替奈普酶静脉溶栓（“舱内溶栓”）模式治疗急性非大血管闭塞性脑梗死的安全性及有效性，为下一步高质量的多中心、大样本、随机对照研究积累经验，为优化早期卒中救治流程和提高预后提供临床治疗新思路。  

Objectives of Study：

Aims to evaluate the safety and efficacy of tenecteplase intravenous thrombolysis administered within a hyperbaric oxygen chamber ("In-chamber thrombolysis") for acute non-large vessel occlusion ischemic stroke. The findings will provide preliminary data for future high-quality multicenter, large-sample randomized controlled trials, while offering novel clinical insights to optimize early stroke treatment protocols and improve patient outcomes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄在18～80岁；②发病时间在3h或4.5h之内；③急诊CT平扫排除颅内出血，考虑急性缺血性卒中；④经CTA/MRA排除颅内大血管闭塞（大血管定义为：颅外段及颅内段在内的颈内动脉、椎动脉V1-V4段、基底动脉、大脑后动脉P1段、大脑前动脉A1段、大脑中动脉M1、M2段）；⑤NIHSS评分5～20之间；⑥本人或家属同意参与本研究

Inclusion criteria

1.Aged 18-80 years; 2.Symptom onset within 3 or 4.5 hours; 3.Non-contrast CT excluding intracranial hemorrhage, suggesting acute ischemic stroke; 4.CTA/MRA confirming absence of large vessel occlusion(Defined as: extracranial/intracranial internal carotid artery, vertebral artery (V1-V4 segments), basilar artery, P1 segment of posterior cerebral artery, A1 segment of anterior cerebral artery, and M1/M2 segments of middle cerebral artery); 5.NIHSS score 5-20; 6.Informed consent obtained from patient or legally authorized representative.

排除标准：

①存在静脉溶栓禁忌症；②存在高压氧治疗禁忌症；③不同意静脉溶栓及急诊入舱治疗；④存在严重肝肾功能障碍；⑤存在恶性肿瘤者；⑥病前mRS评分≥3分；⑦不愿签署知情同意书者

Exclusion criteria：

1.Contraindications to intravenous thrombolysis; 2.Contraindications to hyperbaric oxygen therapy (HBOT); 3.Refusal of either intravenous thrombolysis or emergency HBOT; 4.Severe hepatic or renal dysfunction; 5.Active malignancy; 6.Pre-stroke modified Rankin Scale (mRS) score >=3; 7.Unwillingness to provide informed consent.

The investigator shall promptly, accurately, completely, and legibly transcribe the data from the subjects' original observation records into the case report forms (CRFs). The data entry shall be performed using a double-data-entry system (dual-person, dual-computer entry), followed by two rounds of database verification. Electronic data files shall be categorized and stored, with multiple backups saved on separate disks or storage media to ensure proper preservation and prevent damage.
