本研究旨在探究完全腹腔镜下全胃切除食管空肠“叠契式”手工吻合对比器械吻合的安全性和可行性  

Objectives of Study：

This study aims to investigate the safety and feasibility of the Folded-layered manual esophagojejunal anastomosis versus stapled anastomosis in totally laparoscopic total gastrectomy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.术前胃镜及病理学检查确诊为胃癌或胃食管结合部腺癌（Siemert II/III型）； 2.临床分期为cT2-4aN+M0，行完全腹腔镜下全胃切除术； 3.接受新辅助化疗或免疫治疗后临床疗效明显者； 4.临床病理资料完整； 5.所有手术均获患者充分知情同意，并签署知情同意书。

Inclusion criteria

?1.Preoperative diagnosis? confirmed by gastroscopy and pathological examination as ?gastric cancer? or ?adenocarcinoma of the esophagogastric junction (Siewert type II/III)?. ?2.Clinical stage? classified as ?cT2-4aN+M0? and planned for ?totally laparoscopic total gastrectomy?. 3.Patients who demonstrated ?significant clinical response? after ?neoadjuvant chemotherapy? or ?immunotherapy?. ?4.Complete and available clinicopathological data?. ?5.Written informed consent? was obtained from all patients prior to surgery.

排除标准：

1.年龄<18岁或>75岁； 2.患者因出血、穿孔、梗阻等行急诊手术； 3.术中或术后病理证实为T4b或M1； 4.术中联合脏器切除或姑息切除； 5.术前放疗的患者； 6.ASA分级>Ⅲ级。

Exclusion criteria：

?1.Age <18 or >75 years? 2?.Emergency surgery? for complications (e.g., hemorrhage, perforation, obstruction) ?3.Pathologically confirmed T4b (invading adjacent organs)? or ?M1 (distant metastasis)? intraoperatively/postoperatively ?4.Combined organ resection? or ?palliative resection? during surgery 5.History of ?preoperative radiotherapy? ?5.ASA physical status classification > Class III
