研究目的：

纳入我院100名全膝关节置换术患者，随机分为髌下脂肪垫切除组，和内侧带蒂髌下脂肪垫皮瓣组。研究将通过全膝关节置换术中脂肪垫操作差异进行分组干预，术后进行为期1年的随访，对比两组患者的不同指标的结局差异。以此探讨脂肪垫处理方案，为临床操作提供参考。  

Objectives of Study：

A total of 100 patients undergoing total knee arthroplasty in our hospital were randomly divided into subpatellar fat pad resection group and subpatellar fat pad flap group with medial pedicle. The study will be based on the difference in fat pad operation during total knee replacement to group intervention, and a 1-year follow-up after surgery to compare the outcomes of different indicators between the two groups of patients. The treatment scheme of fat pad was discussed to provide reference for clinical operation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=60岁； 2.术前完善DR、CT检查，明确为膝骨关节炎，且进行全膝关节置换。

Inclusion criteria

1. Age>=60 years; 2. Complete DR and CT examinations before surgery, and confirm that it is knee osteoarthritis, and perform total knee replacement.

排除标准：

1.重度膝内外翻、Paprosky II-III级骨缺损； 2.有严重心血管疾病、神经系统疾病、凝血功能障碍、风湿免疫系统疾病、感染性疾病等可能影响手术及术后恢复的患者； 3.既往有膝关节手术史或严重外伤史的患者。

Exclusion criteria：

1. Severe knee valgus, Paprosky grade II-III bone defect; 2. Patients with severe cardiovascular diseases, nervous system diseases, coagulation disorders, rheumatic immune system diseases, infectious diseases, etc., which may affect surgery and postoperative recovery; 3. Patients with a history of knee surgery or severe trauma in the past.

将髌下脂肪垫的内侧部分作为蒂，外侧部分向外侧翻转，形成一个带蒂的脂肪垫瓣，关节假体置入后将脂肪蒂缝合于胫骨平台前内侧。

The medial part of the subpatellar fat pad was used as the pedicle, and the lateral part was turned over to form a pedicled fat pad flap. After the joint prosthesis was inserted, the fat pedicle was sutured to the medial anterior tibial plateau.
