本实验拟对腹腔镜结肠癌根治术老年患者实施不同的麻醉方案，观察小剂量艾司氯胺酮术中使用对患者TNF-α、IL-6水平的影响、术后镇痛的效果及不良反应，评价艾司氯胺酮对炎症因子的抑制作用，以期为老年腹腔镜结直肠癌手术患者麻醉方案选择提供指导。  

Objectives of Study：

This study was intended to implement different anesthesia schemes for elderly patients undergoing laparoscopic radical resection of colon cancer, observe the effects of low-dose esketamine on the levels of TNF-α and IL-6, postoperative analgesia and adverse reactions, and evaluate the inhibitory effect of esketamine on inflammatory factors, with a view to providing guidance for the selection of anesthesia schemes for elderly patients undergoing laparoscopic colorectal cancer surgery.

药物成份或治疗方案详述：

选择择期行腹腔镜结直肠癌根治术的老年患者60例，年龄60岁-85岁，BMI(18.5-25kg/m2)，ASA分级：I-II级；心功能：1-2级。采用单中心随机双盲试验，麻醉维持期间采用小剂量艾司氯胺酮0.2mg/(kg·h)或生理盐水泵注使用，并以维持患者血流动力学稳定为基准，调整丙泊酚用量，维持血压、心率波动在基础值的±20%以内、BIS在40-60范围，两组术中其他用药和术后镇痛方案相同。 

Description for medicine or protocol of treatment in detail：

Sixty elderly patients, aged 60-85 years, BMI(18.5-25kg/m2), ASA grade I-II, were selected for laparoscopic radical resection of colorectal cancer. Cardiac function: Grade 1-2. A single-center randomized double-blind trial was conducted. During the period of anesthesia maintenance, a small dose of esketamine 0.2mg/(kg·h) or physiological salt pump was used, and the dosage of propofol was adjusted to maintain the hemodynamic stability of patients, so as to maintain the fluctuation of blood pressure and heart rate within +/-20% of the basic value and BIS within the range of 40-60. Other intraoperative drugs and postoperative analgesia were the same in both groups. 

纳入标准：

1、年龄60岁-85岁，BMI(18.5-30kg/m2)，腹腔镜辅助结肠癌根治或者直肠癌根治术；2、常规检查基本正常(肝肾功能、凝血功能、血常规、心电图、胸部CT)，3、ASA分级：I-II级；心功能：1-2级。

Inclusion criteria

1.Aged 60 to 85 years old, BMI(18.5-30kg/m2), laparoscopic assisted radical resection of colon cancer or rectal cancer; 2.Routine examination was basically normal (liver and kidney function, coagulation function, blood routine examination, electrocardiogram, chest CT),;3.ASA grade: I-II; Cardiac function: Grade 1-2.

排除标准：

1、精神病患者。 2、艾司氯胺酮用药史及过敏者，或应用镇静药物及药物依赖者。 3、有感染性疾病者。 4、正参加其它临床试验的患者。 5、脊柱侧弯畸形，无法实施硬膜外穿刺操作。

Exclusion criteria：

1.Mental patients. 2. Those with a history of using esketamine and allergies, or those with sedative drugs and drug dependence. 3.People with infectious diseases. 4.Patients who are participating in other clinical trials. 5.scoliosis deformity, unable to perform epidural puncture operation..

麻醉过程所有药物配置需实验方案设计者完成（针剂均标：实验用药），放入密闭信封中，麻醉实施前取出，麻醉实施及随访需实验实施者完成，两个麻醉医生之间不沟通，麻醉实施者、随访者及患者不知道分组情况。
