本研究旨在探索一项以社区衰弱老年人为中心、基于社会支持理论的干预措施，通过数字化的综合干预模式和专业康复医生和护士指导干预练习，以探索一套有效的社区衰弱老年人的干预策略。  

Objectives of Study：

The aim of this study is to explore an intervention for community-dwelling frail older adults, based on the theory of social support, through a digitised integrated intervention model and guided intervention exercises by professional rehabilitation doctors and nurses, to explore a range of effective intervention strategies for community-dwelling frail older adults.

纳入标准：

1. 之前参与了基线筛查并同意后续联系 2. 年龄在65岁及以上
 3. 目前居住在服务社区，并计划在未来6个月内留在该地区 4. 能够独立行走，无需他人帮助或使用拐杖 5. 能够用普通话和南通方言等主要官方语言进行沟通
 6. 医疗状况稳定（非急性期）
排除标准
 7. 签署了知情同意书

Inclusion criteria

1.Previous participation in the baseline screening and consent provided for subsequent contact 2.Individuals aged 65 years and above 3.Currently living in the community served and planning to remain in the area for the next 6 months . 4.Able to walk independently without assistance from others or the use of crutches 5.Able to communicate in one of the predominant official languages spoken in the Mandarin and Nantong dialect 6.Stable medical condition (non-acute phase)Exclusion Criteria 7.Signed the informed consent form

排除标准：

1. 自报的中风、关节炎、帕金森病、严重肺炎或重大心脏病（抑郁症、双相障碍）病史 2. 影响沟通的严重视力障碍 3. 参与类似体育活动计划 4. 患有终末期疾病、住院或近期有亲人去世 5. 由当地研究医生确定的不适宜安全参与试验活动的禁忌症

Exclusion criteria：

1.Self-reported history of stroke, arthritis, Parkinson’s disease, severe pneumonia, or significant heart disease (depression, bipolar, disorder) 2.Severe visual impairment that affects communication 3.Participation in a similar physical activity program 4.Presence of terminal illness, hospitalization, or recent death 5.Contraindications to safely engage in trial activities as determined by local study physicians

Intervention：

The IIG will receive standardised integrated intervention video courses in the health centres of each community and will be led by rehabilitation physicians and professional nurses. The standardised integrated intervention video courses will encompass four key components, and will be conducted twice a week, with each session lasting 50 to 60 minutes. Warm-Up (10 minutes): Participants will engage in gentle warm-up exercises to prepare for the main training activities. Physical Exercise (20 minutes): This segment will focus on joint activities involving the shoulder, elbow, hip, knee, and ankle, as well as muscle strength training specifically designed for older adults. Cognitive Training (10 minutes): Activities will include alternating finger movements (e.g., touching a clock, playing rock-paper-scissors, and alternately touching the nose and earlobes with both hands) and picture-based speech exercises, such as describing images. Oral Training (3 minutes): Participants will perform movements involving the upper jaw, cheeks, and lips, including the pronunciation of sounds like "pa-ta-ka-ra." Urinary Incontinence Prevention (3 minutes): Exercises will include the Hip Bridge, lower extremity muscle training, and pelvic floor muscle training. Reducing Social Isolation (4 minutes): This component will involve attention training and forehead tapping exercises. The session will conclude with relaxation techniques, a course summary, and health education. Each participant in the intervention group will receive a booklet containing information on nursing care for common chronic diseases associated with aging, guidelines for a balanced nutritional diet, and recommendations for moderate physical activity.
