研究目的：

本研究拟建立以社区为基础，综合多学科（心血管、内分泌、心理、营养、全科）对慢性心力衰竭进行早期干预，以探索符合上海社会发展需要的心力衰竭疾病管理模式，减少有症状心力衰竭发生，改善患者预后和生活质量，降低医疗费用。  

Objectives of Study：

This study intends to establish a community-based, multi-disciplinary (cardiovascular, endocrinology, psychology, nutrition, general medicine) early intervention for chronic heart failure, in order to explore a heart failure disease management model that meets the needs of social development in Shanghai, reduce the occurrence of symptomatic heart failure, improve patient prognosis and quality of life, and reduce medical costs. This study intends to establish a community-based, multi-disciplinary (cardiovascular, endocrinology, psychology, nutrition, general medicine) early intervention for chronic heart failure, in order to explore a heart failure disease management model that meets the needs of social development in Shanghai, reduce the occurrence of symptomatic heart failure, improve patient prognosis and quality of life, and reduce medical costs.

35 岁-85 岁符合 ACC/AHA 分期 A 期和 B 期无症状 HF 患者。

Inclusion criteria

Patients aged 35-85 years with asymptomatic HF who meet ACC/AHA stages A and B.
