研究目的：

评估正镜直视逐层剥离法在消化道黏膜及黏膜下病变微创治疗中的安全性、有效性及技术优势，明确其临床价值。  

Objectives of Study：

To evaluate the safety, efficacy and technical advantages of the orthoscopic direct vision layer-by-layer peeling method in the minimally invasive treatment of gastrointestinal mucosal and submucosal lesions, and to clarify its clinical value.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁，性别不限； 2.内镜下确诊直肠病变，符合 ESD 适应征； 3.内镜下确诊食管、胃黏膜病变，符合 ESD 适应症； 4.消化道黏膜下肿瘤符合内镜治疗 SMT 的适应证； 5.耐受全麻麻醉； 6.纳入研究者均知晓本研究并签署知情同意书

Inclusion criteria

1.Age >=18 years old, gender-neutral; 2.endoscopically confirmed rectal lesions that meet the indications for ESD; 3.endoscopically confirmed esophageal and gastric mucosal lesions that meet the indications for ESD; 4.submucosal tumors of the gastrointestinal tract that meet the indications for endoscopic treatment of SMT; 5.tolerance of general anesthesia anesthesia; 6.inclusion in the study was informed of the study and signed an informed consent form.

排除标准：

1.月经期、妊娠期、哺乳期妇女； 2.既往胃、结直肠外科手术史； 3.合并心脑血管、肺、肝、肾等严重器质性疾病者； 4.合并血液病、凝血功能障碍者； 5.合并炎症性肠病病者； 6.术前1周内服用抗凝药物史者； 7.在四周内注册参加其他临床试验。 8.符合内镜下切除 SMT 的禁忌证：①明确发生淋巴结或远处转移的病变。②对于部分明确发生淋巴结或远处转移的SMT，为获取病理需大块活检，可视为相对禁忌证。③经过详细的术前评估，确定为一般情况差、无法耐受内镜手术者。

Exclusion criteria：

1.Menstruating, pregnant, breastfeeding women; 2.history of previous gastric and colorectal surgery; 3.people with serious organic diseases such as cardiovascular, cerebrovascular, pulmonary, hepatic, renal; 4.people with hematologic diseases and coagulation disorders; 5.people with inflammatory bowel disease; 6.people with history of anticoagulant medication taken within one week before the operation; 7.people who are enrolled in other clinical trials within four weeks. 8.Contraindications for endoscopic resection of SMT: (1). For lesions with definite lymph node or distant metastasis. (2)For some SMT with definite lymph node or distant metastasis, large biopsy is required to obtain pathology, which can be regarded as a relative contraindication.;(3)After detailed preoperative evaluation, it is determined that the general condition is poor and cannot tolerate endoscopic surgery.
