研究目的：

1. 基于多理论模型（MTM），识别社区老年 2 型糖尿病患者身体活动的影响因素。 2. 分析不同影响因素之间的相互关系及其对身体活动水平的综合作用。 3. 为后续制定基于MTM的社区老年2型糖尿病患者身体活动干预方案提供科学依据。  

Objectives of Study：

1. Based on the Multi-Theory Model (MTM), identify the influencing factors of physical activity among community-dwelling elderly patients with type 2 diabetes. 2. Analyze the interrelationships among different influencing factors and their combined effects on the physical activity levels. 3. Provide scientific basis for the subsequent formulation of intervention plans for physical activity among elderly patients with type 2 diabetes in the community based on MTM.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)年龄≥60 岁； (2)临床确诊为 2 型糖尿病，病程≥1 年； (3)在本社区居住≥6 个月； (4)意识清楚，能够理解并配合研究调查。

Inclusion criteria

1. Age >=60 years old; 2.Clinically diagnosed type 2 diabetes mellitus with a course of disease >=1 year; 3.Have lived in the community for >=6 months; 4.Clear awareness, able to understand and cooperate with research investigations.

排除标准：

(1)患有严重心、脑、肾等重要脏器功能障碍，无法进行基本身体活动者； (2)患有精神疾病或认知障碍，影响问卷调查及数据收集者； (3)近期（3 个月内）有重大创伤、手术或急性感染病史。

Exclusion criteria：

1.Patients with serious dysfunction of heart, brain, kidney and other important organs, unable to carry out basic physical activities; 2.People with mental illness or cognitive impairment, which affects the questionnaire and data collection; 3.Recent (within 3 months) history of major trauma, surgery or acute infection.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.数据采集：通过问卷星的形式发放电子问卷调查，问卷收集患者的一般情况、运动行为改变得分、糖尿病压力得分、糖尿病患者自我效能得分和社会支持得分；2.数据管理：通过问卷星电子版问卷直接将数据导入SPSS25.0进行数据统计分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data collection: An electronic questionnaire was issued in the form of questionnaire star. The questionnaire collected general demographic data, changes for physical activity, pressure level, self-efficacy, and social support level; 2.
