研究目的：

（1）调查老年患者院外带入压力性损伤的现状，分析老年患者院外带入压力性损伤影响因素； （2）建立并验证老年患者院外带入压力性损伤的预测模型，开发预测工具，以便家庭照护人员和社区工作人员能够早期识别压力性损伤高危人群，采取有针对性的干预措施，降低压力性损伤发生风险，提高老年人的生活质量。  

Objectives of Study：

(1) Investigate the current situation of pressure injuries brought in by elderly patients outside the hospital, and analyze the influencing factors of pressure injuries brought in by elderly patients outside the hospital; (2) Establish and validate a prediction model for pressure injuries brought in by elderly patients outside the hospital, develop prediction tools, so that family caregivers and community workers can identify high-risk groups for pressure injuries early, take targeted intervention measures, reduce the risk of pressure injuries, and improve the quality of life of the elderly.

纳入标准：

1.年龄>=60岁老年人； 2.病情许可，无沟通障碍；愿意接受问卷调查。

Inclusion criteria

1. Age> = 60 years old; 2. Condition permitted, no communication barrier; Willing to take surveys.

排除标准：

难以观察到皮肤完整性的严重皮肤病或皮肤损伤

Exclusion criteria：

Serious skin diseases or skin injuries that make it difficult to observe skin integrity.
