主要目的：评价 LISA vs. INSURE技术给予牛肺表面活性物质在早产儿中的真实世界有效性。 次要目的： 1. 探索LISA vs. INSURE技术给予牛肺表面活性物质治疗有效性的相关影响因素； 2. 评价 LISA vs. INSURE技术给予牛肺表面活性物质治疗在早产儿中的安全性。  

Objectives of Study：

Primary Objective: To evaluate the real-world effectiveness of administering calf pulmonary surfactant using LISA vs. INSURE techniques in preterm infants. Secondary Objectives: 1. To explore factors influencing the effectiveness of treatment with calf pulmonary surfactant administered via LISA vs. INSURE techniques. 2. To assess the safety of administering calf pulmonary surfactant using LISA vs. INSURE techniques in preterm infants.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.胎龄在25-32周的早产儿； 2.出生后产房及转运至新生儿监护室过程中持续无创辅助呼吸，且招募时未进行有创机械通气； 3.具有肺表面活性物质治疗的临床指征，符合以下条件之一：经鼻持续气道正压通气（nCPAP）压力>=6cmH2O、吸入氧气浓度（FiO2）>0.30、影像学确认呼吸窘迫综合症（RDS）或病情进展快，需要机械通气； 4.拟采用LISA或INSURE技术给予牛肺表面活性物质治疗； 5.患儿监护人同意参与研究。

Inclusion criteria

1. Preterm infants with gestational age of 25-32 weeks; 2. Continuous non-invasive assisted breathing in the delivery room and during transfer to the neonatal intensive care unit after birth, and no invasive mechanical ventilation was performed at the time of recruitment; 3. Have a clinical indication for pulmonary surfactant therapy, meet one of the following conditions: nasal continuous positive airway pressure (nCPAP) pressure >=6cmH2O, inspired oxygen concentration (FiO2) >0.30, imaging confirmed respiratory distress syndrome (RDS) or rapid disease progression, requiring mechanical ventilation; 4. It is proposed to use LISA or INSURE technology to give surfactant treatment to bovine lung; 5. The child's guardian agrees to participate in the study.

排除标准：

1.严重先天性畸形； 2.影响全身血流动力学的严重紫绀型先天性心脏病； 3.先天性遗传性代谢性疾病； 4.胎粪吸入所致呼吸窘迫； 5.严重感染。

Exclusion criteria：

1. Severe congenital malformations; 2. Severe cyanotic congenital heart disease affecting systemic hemodynamics; 3. Congenital hereditary metabolic diseases; 4. Respiratory distress caused by meconium aspiration; 5. Severe infection.
