研究目的：

主要目的： 利用超声斑点追踪技术对脓毒症患者颈动脉弹性进行定量评估，探究该技术在早期发现血管弹性改变中的可行性及临床价值。 次要目的： 分析颈动脉应变（Circumferential Strain, CS）及应变率（Circumferential Strain Rate, CSR）等指标与常规超声参数（管腔直径变化等）的关系。 探讨脓毒症患者颈动脉弹性改变与疾病严重程度（如SOFA评分、APACHE II评分等）和预后的相关性。  

Objectives of Study：

Primary Objective: To quantitatively evaluate carotid arterial elasticity in sepsis patients using ultrasound speckle tracking, and to explore the feasibility and clinical value of this technique for early detection of vascular elastic changes. Secondary Objectives: ? To analyze the relationships between carotid parameters—specifically circumferential strain (CS) and circumferential strain rate (CSR)—and conventional ultrasound measurements (e.g., changes in luminal diameter). ? To investigate the correlation between alterations in carotid elasticity and disease severity (e.g., SOFA score, APACHE II score) as well as patient prognosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

非脓毒症组： (1) 年龄 ≥ 18岁； (2) APACHE II评分 >8分； (3) 无感染证据； (4) 无明显器官功能损伤证据，主要以外科大手术（普外科手术、骨科手术）患者为主； (5) 生命体征稳定，未使用血管活性药物； (6) 自愿参与本研究并签署知情同意书。 脓毒性休克组 (1) 年龄 ≥ 18岁； (2) 符合最新脓毒症3.0诊断标准（Sepsis-3）； (3) 处于急性期的脓毒性休克患者； (4) 自愿参与本研究并签署知情同意书。

Inclusion criteria

Non-septic Group (1) Age >= 18 years; (2) APACHE II score >8 points; (3) No evidence of infection; (4) No obvious evidence of organ dysfunction, primarily consisting of patients undergoing major surgical procedures (e.g., general surgery, orthopedic surgery); (5) Hemodynamically stable without the use of vasoactive agents; (6) Voluntary participation with signed informed consent. Septic Shock Group (1) Age >= 18 years; (2) Meeting the Sepsis-3 diagnostic criteria; (3) Patients in the acute phase of septic shock; (4) Voluntary participation with signed informed consent.

排除标准：

(1) 既往明确诊断存在明显的颈动脉粥样硬化斑块（如IMT >1.2 mm或斑块形成）且病情严重； (2) 合并严重心肌病、心律失常、先天性心脏病等可能显著影响血流动力学的情况； (3) 合并活动性肝肾功能不全或其他严重全身性疾病（如恶性肿瘤晚期）； (4) 超声图像质量不佳，无法得到可靠的斑点追踪分析结果； (5) 妊娠或哺乳期女性（因激素水平改变可能影响血管状态）； (6) 研究过程中退出或其他原因无法配合完成随访。

Exclusion criteria：

(1) A prior clear diagnosis of significant carotid atherosclerotic plaques (e.g., IMT > 1.2 mm or plaque formation) along with severe condition; (2) Concomitant severe cardiomyopathy, arrhythmia, congenital heart disease, or other conditions that may significantly affect hemodynamics; (3) Concomitant active hepatic or renal failure or other severe systemic diseases (e.g., advanced malignant tumors); (4) Poor ultrasound image quality that precludes reliable speckle tracking analysis; (5) Pregnant or lactating women (as hormonal changes may affect vascular status); (6) Withdrawal from the study or inability to complete follow-up for other reasons.

Continuous ultrasound video clips of 3–5 cardiac cycles are acquired from the short-axis view on the same side, while the patient’s blood pressure and heart rate are recorded.
