本研究拟探讨复发性HTGP中的肠道菌群微生物特征，并探索菌群移植后对降低复发性HTGP的作用及预防前景，设计更精确的微生物防治策略。  

Objectives of Study：

This study aims to explore the microbial characteristics of intestinal microbiota in recurrent HTGP, and explore the effect of microbiota transplantation on reducing recurrent HTGP and its prevention prospects, so as to design more precise microbial control strategies.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

满足复发性高脂血症急性胰腺炎患者，AP诊断根据《急性胰腺炎分类—2012年：国际共识修订亚特兰大分类和定义》，急性胰腺炎的诊断标准至少满足以下两项：（1）上腹部持续性腹痛；（2）血清淀粉酶和/或脂肪酶超过正常范围上限3倍；（3）腹部影像学结果支持急性胰腺炎的影像学改变（增强CT、MRI、腹部超声、EUS等）。根据《急性胰腺炎分类-2012年：国际共识修订亚特兰大分类和定义》评价的严重程度。HTGP诊断标准：（1）首先符合《2012年美国亚特兰大急性胰腺炎新分级、分类系统》中的AP诊断标准。（2）其次合并高甘油三酯血症（HTG）〔血清TG水平≥1 000 mg/dl（11.30 mmol/L）〕，或血清TG水平介于500~1000 mg/dl（5.65~11.30 mmol/L）但血清呈乳糜状。同时符合（1）和（2）且排除AP的其他病因如胆管疾病、酒精、创伤、肿瘤等，则HTG-AP诊断成立。RAP定义为AP发作次数≥2次，无胰腺内外分泌功能障碍、纤维化、钙化等永久组织学改变，且2次发作时间间隔≥3个月。

Inclusion criteria

Meet patients with acute pancreatitis with recurrent hyperlipidemia:AP Diagnosis According to the Classification of Acute Pancreatitis-2012: International Consensus Revision of the Atlanta Classification and Definition, the diagnostic criteria for acute pancreatitis are met by at least two of the following: (1) persistent abdominal pain in the upper abdomen; (2) serum amylase and/or lipase exceeding three times the upper limit of the normal range; and (3) abdominal imaging findings that support imaging changes in acute pancreatitis ( enhanced CT, MRI, abdominal ultrasound, EUS, etc.). Severity evaluated according to the Classification of Acute Pancreatitis-2012: International Consensus Revised Atlanta Classification and Definitions.Diagnostic criteria for HTGP: (1) Firstly, meet the diagnostic criteria for AP in the New Grading, Classification System for Acute Pancreatitis, Atlanta, GA, 2012. (2) Secondly, combined with hypertriglyceridemia (HTG) [serum TG level >= 1,000 mg/dl (11.30 mmol/L)], or serum TG level between 500 and 1,000 mg/dl (5.65 and 11.30 mmol/L) but with celiac disease. The diagnosis of HTG-AP was established if both (1) and (2) were met and other etiologies of AP such as bile duct disease, alcohol, trauma, and tumor were excluded.RAP was defined as >= 2 episodes of AP without permanent histologic changes such as pancreatic endo- and exocrine dysfunction, fibrosis, or calcification, and the interval between the 2 episodes was >= 3 months.

排除标准：

未满18周岁、妊娠或哺乳哺乳期女性；腹痛至入院时间>48h；合并肝硬化、慢性肾功能衰竭、自身免疫性疾病、血液系统疾病、恶性肿瘤、免疫缺陷综合征等疾病；入院时合并糖尿病酮症酸中毒。

Exclusion criteria：

Females under the age of 18, pregnant or breastfeeding; Abdominal pain to >48 hours from the time of admission; Combined with liver cirrhosis, chronic renal failure, autoimmune diseases, hematologic diseases, malignant tumors, immunodeficiency syndromes and other diseases; Concomitant diabetic ketoacidosis on admission.

予以肠道菌群移植，肠道菌群移植给予患者聚乙二醇4000散1包，一天两次轻泻肠道准备，利福昔明0.2，一天两次，共3天，停药3天口服菌群胶囊6粒/天，5天为一疗程。共3个疗程，每个疗程间隔1周进行。

Intestinal flora transplantation was given, and the patient was given 1 package of polyethylene glycol 4000 powder for intestinal flora transplantation, laxative intestinal preparation twice a day, rifaximin 0.2, twice a day, a total of 3 days, 3 days of oral microbiota capsules 6 capsules/day, 5 days for a course of treatment. There are 3 courses in total, and each course is performed 1 week apart.
