研究目的:

通过视网膜下注射抗VEGF药物和玻璃体腔注射抗VEGF药物的干预进行对比,随访并评估患者最佳矫正视力、微视野、光学相干断层扫描及血流成像,以明确视网膜下注射抗VEGF药物的有效性和安全性,证实视网膜下注射抗VEGF药物可以有效治疗难治性糖尿病性黄斑水肿,相较于玻璃体腔注药可以减少注药次数,减轻患者经济负担,降低手术创伤和感染风险,为TDME的治疗提供更多的诊疗方案和依据。  

Objectives of Study:

The study compares the interventions of subretinal injection of anti-VEGF agents and intravitreal injection of anti-VEGF agents. Patients are followed up and assessed in terms of best-corrected visual acuity (BCVA), microperimetry, optical coherence tomography (OCT), and OCT angiography. This is done to confirm the efficacy and safety of subretinal injection of anti-VEGF agents, to demonstrate that subretinal injection of anti-VEGF agents can effectively treat refractory diabetic macular edema (DME). Compared with intravitreal injection, subretinal injection can reduce the number of injections, alleviate the economic burden on patients, and lower the risks of surgical trauma and infection. This study provides more therapeutic options and evidence for the treatment of refractory DME.

纳入标准:

患者因糖尿病性视网膜病变、玻璃体积血合并牵引性视网膜脱离行玻璃体切割及硅油注入术后,并连续规律接受3次抗VEGF治疗后仍存在DME的患者,黄斑中央凹视网膜厚度(CRT)>300?um。

Inclusion criteria

Patients with diabetic retinopathy complicated by vitreous hemorrhage and tractional retinal detachment who underwent vitrectomy and silicone oil injection, and still had diabetic macular edema (DME) after three consecutive and regular anti-VEGF treatments, with a central retinal thickness (CRT) of the macula >300 μm.

排除标准:

屈光介质明显浑浊无法获得术前光学相干断层扫描(OCT)图像,既往PPV手术史、明显的RPE萎缩、活动性或高危型增殖性糖尿病视网膜病变、牵拉性视网膜脱离、新生血管性青光眼和独眼患者。

Exclusion criteria:

Patients with significant opacification of the ocular media, making it impossible to obtain preoperative optical coherence tomography (OCT) images, a history of pars plana vitrectomy (PPV) surgery, significant retinal pigment epithelium (RPE) atrophy, active or high-risk proliferative diabetic retinopathy, tractional retinal detachment, neovascular glaucoma, and those with monocular vision.

The investigator will generate random numbers for the subjects and randomly assign them to the subretinal injection group and the intravitreal injection group at a 1:1 ratio using an interactive web-based response system.
