研究目的:

了解老年人流感、肺炎和带状疱疹3种疫苗的接种情况,对3种疫苗的态度,疫苗犹豫影响因素,研制适合我国老年人的“多媒体疫苗处方”,开发智能疫苗处方的数字化技术与软件工具,应用于基层医疗卫生机构,并评价该技术对老年人疫苗犹豫和接种行为的社区干预效果,为促进我国老年人群(特别是老年慢性病人群)的疫苗接种,提供科学适宜的方法、路径与工具。  

Objectives of Study:

Understanding the vaccination coverage and attitudes towards influenza, pneumonia, and shingles vaccines among the elderly, as well as the factors influencing vaccine hesitancy. This includes the development of a 'multimedia vaccination prescription' tailored to the elderly in China, along with the creation of digital technology and software tools for intelligent vaccination prescriptions. These tools will be applied in primary healthcare institutions, with an evaluation of their effectiveness in reducing vaccine hesitancy and improving vaccination behavior through community-based interventions. The ultimate goal is to provide scientifically appropriate methods, pathways, and tools to promote vaccination among the elderly population, especially those with chronic diseases.

纳入标准:

1. 开展调查社区的常住居民(居住超过6个月); 2. 年龄≥60 周岁; 3. 具有独立的文字理解和表达能力; 4. 身体状况及精神情绪能够主动配合。

Inclusion criteria

1. Residents of the surveyed community who have lived in the area for more than 6 months; 2. Aged 60 years and above; 3. Possessing independent reading comprehension and expression abilities; 4. In good physical and mental health, capable of actively cooperating.

排除标准:

1. 经调查者解释后拒绝参与者; 2. 沟通交流有障碍者。

Exclusion criteria:

1. Participants who refuse to participate after the investigator's explanation; 2. Individuals with communication barriers.

课题组设计出“数字化疫苗处方干预系统”,该系统可以根据就诊老年人健康情况生成疫苗接种等级的推荐,以及生成疫苗科普知识和健康教育卡片。当老年人前往社区卫生服务中心进行常规就诊、接受基本公卫项目常规体检和随访时,全科医生根据“数字化疫苗处方干预系统”对符合接种条件的就诊老年人推荐的疫苗接种等级,开具疫苗处方,根据系统生成的健康教育卡片,科普健康科普知识,提醒其在流感、肺炎、带状疱疹高发季节可能面临的患病风险,由老年人自行决定是否要接种接种,若选择接种,则直接携带“疫苗处方”前往预防保健科直接接种。

We designed a "digital vaccine prescription intervention system", which can generate recommendations of vaccination levels according to the health conditions of elderly, as well as vaccine science knowledge and health education cards. When the elderly go to the community-based health service center for routine treatment, routine physical examination and follow-up of basic public health projects, the general practitioner issues a vaccine prescription according to the vaccination level recommended by the "digital vaccine prescription intervention system" for the elderly who meet the vaccination conditions. The health education card generated by the system is used to popularize health science knowledge. Remind them of the risk they may face in the high incidence season of influenza, pneumonia and shingles. It is up to the elderly to decide whether to get vaccinated or not. If they choose to get vaccinated, they should bring the "vaccine prescription" directly to the preventive health department for direct vaccination.

Intervention:

When elderly individuals visit the community health service center for routine consultations, basic public health check-ups, and follow-up care, general practitioners (GPs) assess their personal health status, provide vaccine knowledge education, and remind them of the potential health risks during the peak seasons of influenza and pneumonia. GPs explain the protective effects and benefits of vaccines in preventing diseases. Additionally, for each eligible elderly person, a "vaccination prescription" is issued. Interventions are carried out through two methods: health education cards from the GP and the issuance of vaccination prescriptions. The elderly individuals are then allowed to decide independently whether to receive the influenza or pneumonia vaccines.

In contrast, the control group elderly individuals, during routine consultations, basic public health check-ups, and follow-up visits at the community health service center, do not receive a "vaccination prescription" from the general practitioner, nor do they receive targeted health education through health education cards.

Randomization Procedure (please state who generates the random number sequence and by what method):

The medical personnel conducting the intervention will allocate the clusters (Community Health Service Center) to either the intervention group or the control group based on a pre-generated random number sequence. The specific steps are as follows:1.Generate Random Numbers: A statistician independent of the research team uses statistical software (R) to generate a set of random numbers, ensuring that the numbers are uniformly distributed to achieve fair allocation.2.Set Group Allocation Rules: The proportion of clusters in each group is determined (e.g., 1:1), ensuring that each cluster has an equal chance of being assigned to either the intervention or the control group.3.Allocation Process: Upon the inclusion of clusters, the medical personnel will sequentially consult the random number list according to the order of clusters. Based on the pre-established rules (e.g., even numbers assigned to the intervention group, odd numbers assigned to the control group), each cluster is randomly allocated to the corresponding group.
