研究目的：

這個研究的目的在釐清: 當手術適應症發生時，是否應推遲手術至較大年紀這個臨床爭議，藉由前瞻性對照研究來回答這兩個重要問題：1) 推遲屈膝步態的手術兩年，是否造成身體結構及功能、活動及參與、心理及生活品質等等面向的惡化？2)及時手術和推遲手術是否有不同的術後改變？  

Objectives of Study：

This project aims to clarify the controversial issue that whether deferring surgery to an older age resulting in better surgical outcomes. This prospective comparative study is to answer the 2 critical questions: 1) Whether deferring surgery for knee flexion gait results in more limitation in range of motion, more spasticity, and worse gait deviation index after 2 years? 2) Whether the postoperative outcomes are different between the surgeries performed in time and performed 2 years later?

Description for medicine or protocol of treatment in detail：

 

纳入标准：

這是前瞻性、非隨機分配的對照研究，受試者為7-10歲的痙攣雙邊型具行走能力的腦麻病人，步態站立期之最大伸直角度（或最小屈膝角度）達20度以上，閱讀過相同的治療說明後，由家長決定及時手術或者推遲手術。肉毒桿菌毒素注射為延遲手術時的最佳替代療法，計畫中安排延遲手術的時間為兩年，但受試者可決定隨時接受手術。

Inclusion criteria

It is a prospective two-arm parallel non-randomized controlled study in which surgery is compared with deferring surgery for 2 years in patients with spastic knee flexion gait. The patients in deferring surgery group have botulinum toxin injection that is the best current alternative treatment, and they can undergo surgery as request. Subjects are ambulatory diplegic patients with age 7-10 years, who has knee flexion gait with minimum knee flexion angle 20 degrees or more in mid-stance. Forty patients will be allocated to surgery group or deferring surgery group as parents choices.
