男性乳房发育症（gynecomastia，GYN）是指男性乳房腺体结缔组织异常增生和（或）脂肪组织在局部过度堆积，造成的一侧或双侧胸部呈女性胸部般发育、肥大的临床病症，多发生于男性青春期（12-17岁）或老年期（50-70岁），GYN在年轻男性中发病率高达32%-65%，是男性乳房最常见的疾病。男性乳房发育症患者多无明显不适症状，少数患者可有触痛、胀痛等轻度症状，但突起的胸部外形会给部分患者造成社

研究目的：

比较脂肪抽吸后组织旋切与脂肪抽吸后乳晕小切口切除残余腺体两种手术方法治疗腺体脂肪型Simon I-IIb度男性乳房发育症的疗效及手术安全性。  

Objectives of Study：

To explore the clinical outcomes of tissue rotation after liposuction versus areolar incision for glandular tissue removal in the treatment of Gynecomastia

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-60岁之间； 2.男性乳房发育症Simon分度：Simon I度或Ⅱ度者； 3.术前行乳房触诊、彩超、磁共振(MRI)检查报告均为GYN； 4.病程超过12个月，且无自行消退迹象； 5.根据乳房MRI检查确定为腺体脂肪型(腺体量占乳房总体积25%-75%)； 6.外阴生殖器发育无异常、性激素检测正常； 7.自愿加入该临床研究，随机选择脂肪抽吸后组织旋切或脂肪抽吸后乳晕小切口切除残余腺体两种手术方法之一进行治疗，术后资源接受临床研究预定的随访观察，对本研究表示知情，并签署知情同意书。

Inclusion criteria

1. Between 18-60 years old; 2. Simon scale of gynecomastia: Simon I or II; 3. Preoperative breast palpation, color ultrasound, and magnetic resonance (MRI) examination reports are all GYN; 4. The course of the disease is more than 12 months, and there is no sign of spontaneous regression; 5. Determined to be glandular fat type according to breast MRI examination (glandular volume accounts for 25%-75% of the total breast volume); 6. There is no abnormality in the development of the vulva genitals, and the sex hormone test is normal; 7. Voluntarily join the clinical study, randomly select one of the two surgical methods of tissue atherectomy after liposuction or areolar small incision excision of residual glands after liposuction for treatment, and the postoperative resources will be subject to follow-up observation scheduled by the clinical study, and they will be informed about this study and sign the informed consent form.

排除标准：

1.合并严重心、肝、肾及血液系统疾病不能手术； 2.术前乳腺查体或影响学提示乳房有除男性乳房发育外其他病变存在，如乳腺可疑结节、乳腺可疑钙化或术后标本病理检查提示恶性等情况的患者； 3.药物性男性乳房发育：因慢性肝炎、肝硬化或长期服用异烟肼、雌激素、抑制雄激素药物、利尿药等引起的乳房异常发育； 4.病理性男性乳房发育：因有垂体肿瘤、性腺功能减退、假两性畸形等造成乳房发育； 5.病程未超过1年，可经药物治疗的男性乳房发育； 6.不愿接受本研究随机分组选择手术方案、不能按时复诊的患者。

Exclusion criteria：

1. Inoperable with severe heart, liver, kidney and blood system diseases; 2. Patients with preoperative breast examination or impact science showing the presence of other lesions in the breast except for gynecomastia, such as suspicious nodules in the breast, suspicious calcification of the breast or malignant in the pathological examination of postoperative specimens; 3. Drug-induced gynecomastia: abnormal breast development caused by chronic hepatitis, liver cirrhosis or long-term use of isoniazid, estrogen, androgen suppression drugs, diuretics, etc.; 4. Pathological gynecomastia: breast development caused by pituitary tumors, hypogonadism, pseudohermaphroditism, etc.; 5. Gynecomastia with a course of less than 1 year and can be treated with medication; 6. Patients who are unwilling to accept the randomization of this study to select the surgical regimen and cannot return to the clinic on time.
