研究目的：

1.2025年完成社区5000名亚临床房颤高危老人的长程心电监测的筛查工作，获取真实世界中高危老年人群中亚临床房颤的检出率。 2.通过随机对照试验设计，结合社区体检和长程心电监测数据，比较个体化的房颤综合管理策略（房颤临床）和常规管理对预防房颤及卒中/TIA发生风险的效果。 3.形成一套针对社区高危亚临床房颤患者的特殊防、筛、诊、治、康综合管理策略。  

Objectives of Study：

1.Complete the screening of 5,000 community-dwelling elderly individuals at high risk for subclinical atrial fibrillation through long-term electrocardiogram monitoring by 2025, in order to obtain the detection rate of subclinical atrial fibrillation in high-risk elderly populations in the real world. 2.Design a randomized controlled trial to compare the effectiveness of an individualized atrial fibrillation comprehensive management strategy versus conventional management in preventing atrial fibrillation and reducing the risk of stroke/TIA, using data from community health checkups and long-term electrocardiogram monitoring. 3.Develop a comprehensive management strategy for special prevention, screening, diagnosis, treatment, and rehabilitation for community-dwelling high-risk subclinical atrial fibrillation patients.

纳入标准：

年龄>75岁，或年龄>65岁且CHA2DS2-VASc评分大于3分

Inclusion criteria

Age>75 years, or age>65 years with a CHA2DS2-VASc score greater than 3

排除标准：

1.当前或既往有房颤、房扑或房性心动过速的诊断; 2.正在接受长期抗凝治疗; 3.接受临终关怀; 4.终末期肾病; 5.诊断为中度或更严重的痴呆; 6.已植入心脏起搏器和/或除颤器; 7.有过敏历史（如对贴片胶黏剂过敏）; 8.已知有转移性癌症。

Exclusion criteria：

1.Current or past diagnosis of atrial fibrillation, atrial flutter, or atrial tachycardia; 2.Currently receiving long-term anticoagulant therapy; 3.Receiving hospice care; 4.End-stage renal disease; 5.Diagnosed with moderate or more severe dementia; 6.Have an implanted pacemaker and/or defibrillator; 7.History of allergies (e.g., adhesive allergy to patches); 8.Known metastatic cancer.
