研究目的：

深入阐明CAR T细胞疗法在淋巴瘤患者中起效的生物学机制，探究不同免疫调节剂对CAR T细胞扩增及发挥持续抗肿瘤功能的影响，评估不同免疫调节剂对增强CAR T细胞疗法在淋巴瘤病人中的临床疗效的可行性。  

Objectives of Study：

To further elucidate the biological mechanism of CAR T cell therapy in lymphoma patients, explore the influence of different immunomodulators on the expansion and anti-tumor function of CAR T cells, and evaluate the feasibility of different immunomodulators to enhance the clinical efficacy of CAR T cell therapy in lymphoma patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 经组织学确认的大B淋巴瘤，包括弥漫大B淋巴瘤，或弥漫大B淋巴瘤继发中枢神经系统侵犯或转化型滤泡淋巴瘤，或原发纵隔大B细胞淋巴瘤，或原发纵隔大B淋巴瘤继发中枢神经系统侵犯，或伴随MYC和BCL2或BCL6易位的高级别淋巴瘤；
2) 复发或难治患者，其定义为未能达到完全缓解（CR）或在标准免疫化疗（例如R-CHOP或类似疗法）后早期复发；
3) 具备可评估病灶（评价标准Lugano2014）；
4) 对于收集剩余肿瘤组织样本和剩余CD19 CAR T细胞产品的研究部分，患者需签署知情同意书，并至少可提供剩余肿瘤组织或剩余CAR T细胞产品中的一项。

Inclusion criteria

1) Histologically confirmed large B-cell lymphoma, including diffuse large B-cell lymphoma, or diffuse large B-cell lymphoma with secondary central nervous system involvement, or transformed follicular lymphoma, or primary mediastinal large B-cell lymphoma, or primary mediastinal large B-cell lymphoma with CNS involvement, or high-grade B-cell lymphoma with MYC and BCL2 or BCL6 translocation;
2) Relapsed or refractory patients, defined as those who failed to achieve complete response (CR) or relapsed early after standard immunochemotherapy (such as R-CHOP or similar therapies);
3) Evaluable lesions (Lugano2014 as evaluation criteria);
4) For the portion that residual tumor tissue samples and residual CD19 CAR T cell products need to be collected, patients are required to sign the Informed Consent Form(ICF) and are available to provide at least one of the two options: the remaining tumor tissue or the residual CAR T cell products.

排除标准：

1) 纳入标准以外的其他非霍奇金淋巴瘤诊断；
2) 首次接受CD19 CART细胞治疗前使用过其他CAR T细胞治疗或其他基因修饰细胞治疗者。

Exclusion criteria：

1) Other diagnosis of non-Hodgkin lymphoma besides inclusion criteria;
2) Patients who have used other CART cell therapy or other gene-modified cell therapy before first receiving CD19 CART cell.
