研究目的：

本研究基于舌拭子标本的结核分枝杆菌复合群核酸检测试剂盒(PCR-荧光探针法)（TB-qPCR）对无痰/少痰肺结核患者诊断进行真实世界临床性能评估，计算其灵敏度、特异性、阳性预测值等临床性能参数。 本研究评估舌拭子在中国情景下的结核病早期筛查的可行性，讨论是否可以成为痰液样本的替代方案，以期优化当前诊疗策略。  

Objectives of Study：

In this study, we evaluated the real-world clinical performance of the Mycobacterium tuberculosis complex group nucleic acid detection kit (PCR-fluorescent probe method) (TB-qPCR) using tongue swab samples for diagnosing sputum-free or sputum-oligo pulmonary tuberculosis patients. Sensitivity, specificity, positive predictive value, and other clinical performance parameters were calculated. This investigation assessed the feasibility of tongue swabs as an early screening tool for TB in the Chinese context and explored their potential as an alternative to sputum samples, aiming to optimize current diagnostic and treatment strategies.

纳入标准：

1）年龄 >=18岁 2）有下述肺结核疑似临床症状：咳嗽、发烧、盗汗、体重减轻、胸痛、咳血、痰中带血等； 3）影像学表现疑似肺结核症状：空洞、斑片、结节及索条影等 4）患者主诉为“无痰”，“少痰”，且在病历中可溯源；或病例中无体现，但因无法取得合格痰标本需要采用诱导痰或肺泡灌洗液进行病原学检测的患者。

Inclusion criteria

1) Age >= 18 years old 2) Have the following suspected clinical symptoms of tuberculosis: cough, fever, night sweats, weight loss, chest pain, coughing up blood, blood in sputum, etc.; 3) Imaging findings of suspected pulmonary tuberculosis symptoms: cavities, plaques, nodules and cord shadows 4) The main complaint of the patient was "no sputum" and "little sputum", and the source could be traced in the medical record; Or patients who are not present in the case, but need to use induced sputum or alveolar lavage fluid for etiological testing due to the inability to obtain qualified sputum specimens.

排除标准：

1）妊娠和哺乳期患者 2）血行播散性结核病、脑膜或中枢神经系统结核病，或合并人免疫缺陷病毒（HIV）感染的肺外结核病 3）样本基本信息不完整，无法溯源； 4）样本收集、保存或运输不当不能用于检测者； 5）复治的肺结核患者样本； 6）单次痰液采集量 >= 1 mL； 7）不愿意或不能提供知情同意 8）研究者认为需要排除的舌拭子样本

Exclusion criteria：

1) Pregnant and lactating patients 2) Hematogenous disseminated tuberculosis, meningeal or central nervous system tuberculosis, or extrapulmonary tuberculosis with human immunodeficiency virus (HIV) infection 3) The basic information of the sample is incomplete and cannot be traced; 4) Improper collection, storage or transportation of samples can not be used for testing; 5) Samples from retreated pulmonary tuberculosis patients; 6) A single sputum collection volume >= 1 mL; 7) Unwillingness or inability to provide informed consent 8) Tongue swab samples considered by the investigator to be excluded

The composite reference standard of mycobacterial culture and Xpert MTB/RIF assay was used as the gold standard to assess the diagnostic accuracy of oral swab analysis.
