这项研究的目标是调查低水平的机械振动(正畸加速器在35赫兹)是否能加速骨组织代谢,从而促进牙齿更快移动,减少实现治疗目标所需的时间,降低患者随访次数,并为临床应用提供基于证据的医学支持。  

Objectives of Study:

the goal of this study is to investigate whether low-level mechanical vibration (orthodontic accelerator at 35 Hz) can accelerate bone tissue metabolism, thereby promoting faster tooth movement, reducing the time required to achieve treatment goals, decreasing the number of patient follow-ups, and providing evidence-based medical support for clinical applications.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.使用无托槽隐形矫治系统; 2.上颌磨牙需要近移者; 3.利用矫治器弹性形变和附件移动牙齿,无种植钉、橡皮圈等辅助牙齿移动装置; 4.患者依从性好,每天矫治器佩戴不少于 20 小时或是能够每天使用加速器20Min; 5.硬腭区域发育良好,腭皱襞清晰; 6. 牙周健康状况良好; 7. 未接受过正畸治疗。

Inclusion criteria

1. Use a bracketless invisible orthodontic system; 2. Those who need proximity for maxillary molars; 3. Use the elastic deformation and attachments of the appliance to move the teeth, and there are no auxiliary tooth moving devices such as implant nails and rubber rings; 4. The patient has good compliance, and can wear the appliance for no less than 20 hours a day or be able to use the accelerator for 20min a day 5. The hard palate area is well developed, and the palatal folds are clear; 6. Good periodontal health; 7. No orthodontic treatment.

排除标准:

1.口腔卫生极差; 2.明显的骨缺失; 3.牙周病进展期; 4.吸烟史; 5.依从性差,不按时复诊,不配合使用正畸加速器、矫治器更替周期及佩戴时长不满足矫治要求者; 6.大量内收前牙的患者,矫治过程中上颚意外损伤,影响数字化三维重叠精度者; 7. 系统性及代谢性疾病可能影响牙齿移动者; 8. 孕妇。

Exclusion criteria:

1. Oral hygiene poor; 2. Significant bone loss; 3. periodontal disease progress; 4. Smoking history; 5. Poor compliance, not on time, do not cooperate to use orthodontic accelerator, appliance replacement cycle and wear time does not meet the requirements of orthodontic; 6. A large number of patients, in the process of orthodontic palate accident injury, affect digital 3 d overlap accuracy; 7. systemic and metabolic diseases may affect teeth mobile 8. pregnant women.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

病例记录表 (CRF) CRF设计:本研究的CRF包括患者的基线特征(如年龄、性别、错颌畸形诊断等)、材料与方法(干预方式、治疗方法等)和测量结果(如牙齿移动速度与效率、磨牙轴轻度,前磨牙扭转率化)。CRF的设计基于研究目的,确保记录所有相关变量。 数据来源:CRF中的数据来源于参与隐形矫治患者符合条件自愿入选试验后测得。数据提取由经过培训的研究人员完成,确保数据的准确性和完整性。 数据提取流程:研究人员通过试验组每14天更换一次矫治器,每天使用三次,每次连续20分钟的低频振动(Hz=35);对照组为常规无托槽隐形矫治器治疗,每14天更换一次矫治器。截至2023年10月复诊前口扫模型重叠对比切牙组,前磨牙组与磨牙组牙齿移动速率,矫治器设计牙齿到达预期位置后口扫计算牙齿移动效率,前牙扭转率、磨牙轴倾度。得到相应数据记录到CRF中。数据提取过程中,研究人员会进行双人核对,确保数据的准确。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Case Report Form (CRF) CRF Design: The CRF for this study includes patients' baseline characteristics (such as age, gender, malocclusion diagnosis, etc.), materials and methods (intervention methods, treatment methods, etc.), and measurement results (such as tooth movement speed and efficiency, molar axis inclination, and premolar torsion rate). The design of the CRF is based on the research objectives, ensuring the recording of all relevant variables. Data Source: The data in the CRF comes from patients participating in invisible orthodontic treatment who meet the criteria and voluntarily join the trial after measurement. Data extraction is performed by trained researchers to ensure the accuracy and completeness of the data. Data Extraction Process: Researchers replace the aligners every 14 days for the trial group, using them three times a day, each for 20 consecutive minutes of low-frequency vibration (Hz=35); the control group receives conventional bracketless invisible orthodontic treatment, replacing the aligners every 14 days. By October 2023, before the follow-up appointment, the overlapping comparison of the scanned models of the incisor group, premolar group, and molar group is performed to measure the rate of tooth movement. After the aligner design reaches the expected position, the scanned models are used to calculate the efficiency of tooth movement, the rate of incisor torsion, and the inclination of the molar axis. The corresponding data are recorded into the CRF. During the data extraction process, researchers conduct double checks to ensure the accuracy of the data.
