研究目的：

(1) 明确穴位按摩调对促进剖宫产术后乳汁分泌的效果； (2) 探索穴位按摩调控关键基因甲基化水平对泌乳的作用机制。  

Objectives of Study：

1. Evaluate the efficacy of SAA in improving lactation outcomes among postpartum women with IBMS. 2. Investigate the epigenetic mechanisms underlying the effects of acupressure, focusing on DNA methylation in key lactation-related genes through animal randomized controlled trial.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18岁； ②自愿母乳喂养； ③产后48小时内诊断为乳汁不足； ④新生儿Apgar评分8-10分； ⑤能讲中文，并能独立完成问卷填写。

Inclusion criteria

1. aged >=18 years; 2. willing to breastfeed; 3. diagnosed with IBMS within 48 hours post-delivery; 4. neonates with Apgar scores >= 8 at 1 and 5 minutes.; 5. able to read and complete questionnaires independently in Chinese.

排除标准：

①合并严重疾病，如妊娠期高血压疾病、心脏疾病、肝肾功能异常、产后出血等； ②乳腺功能异常者，如乳房发育异常等； ③有医学指征不能母乳喂养者。

Exclusion criteria：

1. severe comorbidities (e.g., hypertension, heart disease, liver or kidney diseases, or postpartum hemorrhage); 2. breast abnormalities or contraindications for breastfeeding; 3. concurrent participation in other lactation-related interventions.

除了对照组的措施外：（1）对产后第2天乳汁不足的妇女，行“促乳按摩三要穴”手法教学（揉/捏/推）：每日3次；早、中、晚各 10-15min；自己能承受的强度（以不感到疼痛为宜）。（2）产妇从产后第 2天持续到产后6个月，在家按摩。

Intervention：

(1) For the women with insufficient milk on the second day after delivery, the teaching method of "massage the three essential points for promoting breast milk" (kneading/pinching/pushing) was given 3 times a day; 10-15min in the morning, middle and evening; I can bear the intensity (with no pain is appropriate). (2) Puerpera were massaged at home from the second day after delivery to 6 months after delivery.

Participants in the control group will receive standard postpartum lactation support, including: 1) breastfeeding guidance upon returning to the ward post-delivery; 2) health education from breastfeeding nurses at discharge, along with health education manuals; 3) telephone follow-ups at 42 days and four months post-discharge to inquire about breastfeeding practices.

Randomization method: A computer-generated odd even matching random number table is used to start counting at any position, and the obtained numbers are assigned to consecutive enrolled patients.
