研究目的：

肥胖（obesity）指损害健康的异常或过量脂肪积累，是育龄妇女常见的健康问题。肥胖孕妇更易出现早产，增加先天性胎儿畸形、肩难产、自然和医学指征早产以及死产的风险。肥胖孕产妇往往还患有慢性病,如妊娠期糖尿病等。通过药物和手术治疗肥胖的安全性有待考量。治疗性生活方式干预（Therapeutic Lifestyle Change, TLC）是一种通过改善生活方式治疗和预防慢性病的方法。通过饮食调整、增加身体活动、减轻体重等措施改善患者的健康状况。AI intervention作为一种新的干预手段，结合治疗性生活方式干预可以监控孕妇的健康数据，及时调整治疗方案，提供个性化的生活方式建议。此干预方式方便孕妇在家中接受管理，迅速传递专业支持，满足孕妇在孕期生活方式改变过程中的需求，易于实施。本研究基于人工智能驱动-治疗性生活方式改变的干预项目，对其干预效果、各年龄段人群、不同BMI分级的适配性进行研究，探索能否有效促进超重或肥胖孕妇健康行为、健康结果改善。  

Objectives of Study：

Obesity, defined as an abnormal or excessive accumulation of fat that is detrimental to health, is a common health problem among women of childbearing age. Obese pregnant women are more likely to experience preterm labor and are at increased risk for congenital fetal abnormalities, shoulder dystocia, natural and medically indicated preterm labor, and stillbirth. Obese pregnant women also tend to suffer from chronic diseases, such as gestational diabetes. The safety of treating obesity with medications and surgery needs to be considered. Therapeutic Lifestyle Change (TLC) is an approach to treating and preventing chronic diseases by improving lifestyle. AI intervention, as a new intervention, combined with Therapeutic Lifestyle Change (TLC), can monitor the health data of pregnant women, adjust the treatment plan in time, and provide personalized lifestyle advice. This intervention facilitates the management of pregnant women at home, rapidly delivers professional support, meets the needs of pregnant women in the process of lifestyle change during pregnancy, and is easy to implement. Based on the AI-driven-therapeutic lifestyle change intervention program, this study investigates its intervention effects, the appropriateness of various age groups and different BMI classifications, and explores whether it can effectively promote the improvement of health behaviors and health outcomes in overweight or obese pregnant women.

纳入标准：

1: 年满18周岁或以上 2: 妊娠8-15周的单胎妇女 3: 孕前BMI ≥28.0kg/m2 4: 意识清楚并能够完成实验目标及随访 5: 有电话，或能够通过家人电话进行随访 6: 能够听懂普通话（后续干预措施涉及普通话）

Inclusion criteria

1: Aged 18 or above 2: Women with a single pregnancy at 8-15 weeks of gestation 3: Pre-pregnancy BMI >=28.0kg/m2 4: Conscious and able to complete lab objectives and follow up visits 5: Have a phone, or be able to follow up with a family member on the phone 6: Ability to understand Mandarin (follow-up interventions involve Mandarin)

排除标准：

1: 辅助生育妊娠 2: 卧床休息 3: 糖尿病或妊娠糖尿病诊断 4: 目前未控制的高血压 5: 最近30天内诊断的甲状腺疾病 6: 心血管、癌症、肺或严重胃肠道疾病史 7: 饮食失调或减肥手术史 8: 严重精神疾病 9: 近期情绪或焦虑障碍史 10: 药物或酒精使用障碍 11: 妊娠13周以上 12: 实验中途退出者 13: 在实验过程中出现严重急性疾病需要治疗者

Exclusion criteria：

1: assisted reproduction of pregnancy 2: bed rest 3: Diagnosis of diabetes or gestational diabetes 4: Current uncontrolled hypertension 5: Thyroid disease diagnosed within the last 30 days 6: History of cardiovascular, cancer, pulmonary or severe gastrointestinal disease 7: History of eating disorders or bariatric surgery 8: Serious mental illness 9: Recent history of mood or anxiety disorders 10: Drug or alcohol use disorders 11: More than 13 weeks of gestation 12: Those who dropped out in the middle of the experiment 13: Those who develop a serious acute illness requiring treatment during the course of the experiment
