研究目的：

1. 全面檢視香港獨居和雙居長者的未來照護偏好和準備（包括經濟、情感、工具和個人照護）相關的需求、風險、障礙、資源、經驗和目標； 2. 實施並評估動機式訪談對照護準備干預措施在增強獨居和雙居長者未來照護需求的態度和行為準備方面的有效性； 3. 透過納入具有吸引力和賦權的照片語音方法，加強和完善動機式訪談對未來照護準備的干預措施； 4. 使用四組整群隨機對照試驗，比較動機式訪談結合照片語音干預、僅動機式訪談干預、教育資訊干預和候補名單控制組在加強獨居和雙居長者未來護理準備的有效性； 5. 透過純動機式訪談和動機式訪談結合照片語音干預、教育資訊干預和候補名單控制課程，檢視參與者對未來照護準備的態度和行為變化的基本過程和模式。  

Objectives of Study：

1. To comprehensively examine the needs, risks, barriers, resources, experiences, and goals related to future care preferences and preparation (including finance, health, family life, social participation and long-term care) among older Chinese singletons and doubletons in Hong Kong; 2. To implement and assess the effectiveness of a conventional Motivational Interviewing (MI) care preparation intervention in enhancing attitudinal and behavioural preparedness for future care needs among older singletons and doubletons; 3. To enhance and refine the MI future care preparation intervention by incorporating the engaging and empowering Photovoice approach; 4. To compare the effectiveness of the MI-Photovoice intervention, the MI-only intervention, education information intervention, and waitlist control in enhancing future care preparation of older singletons and doubletons, using a four-arm cluster randomised controlled trial; 5. To examine the underlying processes and patterns of change in participants’ attitudes and behaviours regarding future care preparation through the MI-only and MI- Photovoice intervention, education information intervention, and waitlist control sessions.

纳入标准：

(a) 年齡 55 歲或以上，(b) 會說中文，以及 (c) 獨居，或僅與配偶或伴侶居住在香港的社區內。

Inclusion criteria

(a) aged 55 years and over, (b) Chinese-speaking, and (c) living alone, or living only with a spouse or partner within a community in Hong Kong.

排除标准：

(a) 需要重症監護（即 IADL 平均項目得分低於 2），以及 (b) 存在嚴重的認知、視覺和聽覺障礙，導致無法完成干預和評估。

Exclusion criteria：

(a) requiring intensive care (i.e., with an IADL average item score 2), and (b) having severe cognitive, visual, and auditory impairment that would lead to inability to complete the intervention and assessment.

Motivational Interviewing with Photovoice

The MI–Photovoice enhanced intervention enhanced the MI–only intervention by incorporating Photovoice in the following ways: (a) by encouraging participants to use mobile phones/digital cameras (provided by the project team when necessary) to capture images representing 1) future care needs (i.e., financial, emotional, and instrumental and personal care), and attitude, values, ambivalence, and experiences related to future care preparation, and 2) care options and personal ideal care arrangements between Session 1 and 2; (b) by engaging participants in discussion about their selected photographs from Session 2 to 4 using the adapted SHOWeD approach .

This intervention group will participate in four face-to-face intervention sessions over a period of four weeks. These sessions include: (1) Awareness Arousal Session (within three hours, group-based): The facilitator will establish a strong, productive relationship with the participants, introducing the structure of the intervention (2) Goal Initiation and Information Gathering Session (within three hours, group-based): Participants will engage in a discussion on their future care- related goals. (3) Decision-making Session (within three hours, individual- based): Participants will be guided through the analysis of various care and preparation options, assessing the appropriateness and feasibility of each based on their personal and family dynamics. (4) Concrete Planning and Sustaining Effort Session (within three hours, group-based): Participants will be supported to develop commitment to future care preparation, set SMART (Specific, Measurable, Achievable, Relevant, Timely) goals, and formulate concrete plans.

Participants will watch educational videos during Sessions 1 and 4. They will also engage in self-learning during Sessions 2 and 3 from a range of materials (available in print or digital formats) on future care preparation. To mirror the intervention groups as closely as possible, educational videos will be presented in the first and last group sessions, while the learning materials will be distributed for individual study on a weekly basis for the remaining two weeks. The content of these educational materials, designed with reference to the preparation for future care model and intervention group sessions, ensures practical and theoretical equivalence to the intervention.

Blinding：

To prevent potential assessment bias, the researchers, who are responsible for data collection and analysis, will be blinded to the group randomisation. Following the Recommendations on Regulatory Issues in Cluster Randomized Studies by the SACHRP Committee (Attachment B: Recommendations on Regulatory Issues in Cluster | HHS.gov), we will avoid revealing allocations of interventions to the recruiting staff and the participants to ensure blinding and minimize selection bias. Moreover, data analysts will also be unaware of the randomization process, group allocation or intervention content, recruitment process, and data collection.
