研究目的：

以BCW理论为框架，制定应用于社区老年脑卒中幸存者的具有规范化、个体化特色的老年脑卒中体力活动行为促进方案，促进患者体力活动，探索更加有效的健康管理途径和方法。  

Objectives of Study：

Based on the BCW theory, a standardized and individualized physical activity promotion program for elderly stroke survivors in the community was developed to promote physical activity of patients and explore more effective ways and methods of health management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）诊断为脑卒中，且均经头颅CT或核磁共振证实者。 （2）年龄≥60岁； （3）有残存偏瘫步态缺陷者； （4）处于缺血性或出血性脑卒中亚急性期（脑卒中发病后5-45天）； （5）由其主治医生批准进行体力活动； （6）Berg平衡量表评分>36分者。

Inclusion criteria

(1) patients diagnosed with stroke and confirmed by head CT or MRI; (2) age >=60 years old; (3) patients with residual hemiplegic gait defects; (4) patients in the subacute stage of ischemic or hemorrhagic stroke (5-45 days after stroke onset); (5) physical activity approved by their treating physician; (6) the Berg Balance Scale score >36

排除标准：

（1）既往有影响运动功能的病史，如：恶性肿瘤、下肢关节炎。
（2）参与其他实验接受干预的患者。

Exclusion criteria：

(1) previous medical history affecting motor function, such as malignant tumor and lower limb arthritis. (2) patients who participated in other experiments and received intervention.
