研究目的：

评价嗜酸乳杆菌改善胰岛素抵抗治疗多囊卵巢综合征伴胰岛素抵抗的肥胖患者临床疗效，积累基础数据，为多囊卵巢综合征伴胰岛素抵抗肥胖患者的治疗提供新思路，本研究可能会改善疾病的症状，有助降低体重、协助排卵，或有益于提高多囊卵巢综合征导致不孕症的患者受孕率。  

Objectives of Study：

To evaluate the clinical efficacy of Lactobacillus acidophilus improving insulin resistance in the treatment of obese patients with polycystic ovary syndrome with insulin resistance, accumulate basic data, and provide new ideas for the treatment of obese patients with polycystic ovary syndrome with insulin resistance. This study may improve the symptoms of the disease, help reduce weight, and assist ovulation. It may be beneficial to improve pregnancy rates in patients with infertility caused by polycystic ovary syndrome.

药物成份或治疗方案详述：

嗜酸乳杆菌JYLA（Lactobacillus acidophilus），经冷冻干燥工艺加工而成的冻干菌粉，辅料 包括菌种培养基和保护剂。产品为粉末状，无肉眼可见杂质， 颜色呈白色至淡黄色。可广泛应用于食品、乳制品及功能性保 健品。 

Description for medicine or protocol of treatment in detail：

Lactobacillus acidophilus (Lactobacillus acidophilus), a freeze-dried bacterial powder produced by freeze-drying process, with auxiliary materials including culture medium and protective agent. The product is powdery, no impurities visible to the naked eye, the color is white to light yellow. It can be widely used in food, dairy products and functional health care products.  

纳入标准：

1.符合多囊卵巢综合征合并肥胖症（BMI≥28kg/㎡）和胰岛素抵抗指数（HOMA-IR）大于等于2.69的患者； 2.育龄期女性，年龄18-40岁； 3.３个月内未服用任何影响月经的药物（如：避孕药、激素类或其它影响胰岛素、血糖、性激素和血脂代谢的药物）及未采用相关治疗手段，且无人工流产记录； 4.妇科检查及辅助检查：内生殖器官与外生殖器无器质性病变，同时无其他原发性疾病者； 5.已签署知情同意书者。

Inclusion criteria

1. Patients with PCOS with obesity (BMI >= 28kg/㎡) and insulin resistance index (HOMA-IR) greater than or equal to 2.69; 2. Women of childbearing age, aged 18-40 years; 3. No drugs affecting menstruation (such as contraceptives, hormones or other drugs affecting insulin, blood sugar, sex hormones and lipid metabolism) were taken within 3 months, and no related treatment was used, and no induced abortion was recorded; 4. Gynecological examination and auxiliary examination: there are no organic lesions in the internal and external genitalia, and there are no other primary diseases; 5. Those who have signed an informed consent form.

排除标准：

1.合并其他内分泌疾病，如糖尿病、甲状腺疾病、分泌性激素的肿瘤等； 2.合并严重或不稳定状态的身体疾病，包括肝、肾、心血管、呼吸、神经或血液系统疾病、自身免疫性疾病等、精神病患者； 3.具有血栓栓塞性疾病史或血栓形成倾向； 4.重度吸烟或酗酒妇女； 5.哺乳期或妊娠期女性； 6.其他恶性肿瘤病史妇女； 7.近３个月内使用他汀类药物/抗生素/嗜酸乳杆菌产品/中药/抗雄激素/胰岛素增敏剂或其他己知或怀疑会影响生殖或代谢功能的药物； 8.体质特殊，对药物过敏； 9.除此疾病外还需进行其他方面的治疗，对本研究产生影响，不能按照本研究方案执行的患者； 10.生殖系统存在先天不足不能符合本研究需求的患者。

Exclusion criteria：

1. Combined with other endocrine diseases, such as diabetes, thyroid disease, secreting sex hormone tumors, etc; 2. Physical diseases associated with severe or unstable states, including diseases of the liver, kidney, cardiovascular, respiratory, nervous or blood system, autoimmune diseases, etc., psychiatric patients; 3. Have a history of thromboembolic disease or a tendency to thrombosis; 4. Women who smoke or drink heavily; 5. Lactating or pregnant women; 6. Women with a history of other malignant tumors; 7. Use of statins/antibiotics/Lactobacillus acidophilus products/Chinese medicines/antiandrogens/insulin sensitizers or other drugs known or suspected to affect reproductive or metabolic function within the last 3 months; 8. Special physique, allergic to drugs; 9. Patients who need to be treated in other aspects in addition to the disease, which affects the study and cannot be carried out according to the study protocol; 10. There are congenital deficiencies in the reproductive system that cannot meet the needs of this study.

由南昌大学第二附属医院的独立统计学家从随机数字表中的任意一行任意一列开始，依次读取100个2位随机数，要求单数和双数的随机数个数相等，若不等，从新提取直至相等，依次录于1-100个事先拟定的研究对象的编号下；然后将全部随机数分为单双数两组，单数50个为A组，双数50个为B组;规定A组使用二甲双胍常规治疗，B组使用二甲双胍+嗜酸乳杆菌。

Independent statisticians from the Second Affiliated Hospital of Nanchang University read 100 2-digit random numbers successively from any row or column in the random number table, and required the number of singular and even random numbers to be equal. If they were not equal, they were extracted again until equal, and recorded in sequence under the numbers of 1-100 pre-prepared research objects; Then all the random numbers are divided into two groups of odd and even numbers, singular 50 as group A, even 50 as group B; Conventional treatment with metformin was prescribed for group A, and metformin + Lactobacillus acidophilus was prescribed for group B.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

病例记录表主要由6个部分组成，第一部分为受试者的基本信息，包括姓名、性别、出生年月、联系方式等等；第二部分受试者的纳入标准；第三部分为受试者的排除标准；第四部分为受试者的一些基线资料信息，包括身高、体重、腰围、臀围、月经周期、胰岛素抵抗、肝功能、性激素六项、影像学检查等等；第五部分为受试者用药前后的症状、临床指标情况，如胰岛素抵抗的程度及性激素六项的变化情况，B超卵巢中卵泡生长的变化情况，用药后主要终点和次要终点事件发生的情况，以及处置的情况等等；最后一部分为受试者随访内容的记录，包括患者胰岛素抵抗改善情况，多囊症状好转情况及BMI和排卵率的改善，以及进行疾病复查的时间地点和化验检查结果等等。

The case record form is mainly composed of six parts. The first part is the basic information of the subjects, including name, gender, date of birth, contact information, etc. Part II: Inclusion criteria of subjects; The third part is the exclusion criteria of subjects. The fourth part is some baseline information of the subjects, including height, weight, waist circumference, hip circumference, menstrual cycle, insulin resistance, liver function, sexhormone six items, imaging examination, etc. The fifth part is about the symptoms and clinical indicators of the subjects before and after medication, such as the degree of insulin resistance and the changes in the six items of sex hormones, the changes in the growth of follicles in the ovaries of B-ultrasound, the occurrence of primary and secondary endpoint events after medication, and the treatment situation. The final part is the record of the subjects' follow-up, including the improvement of insulin resistance, the improvement of polycystic symptoms, the improvement of BMI and ovulation rate, and the time and place of disease review and the results of laboratory tests.
