本研究旨在明确糖尿病前期及近期诊断的2型糖尿病患者的肠道菌群变化特点，以及益生菌乳双歧杆菌M8对确糖尿病前期及近期诊断的2型糖尿病患者血糖代谢及认知障碍的疗效及可能作用机制，为2型糖尿病及认知障碍的防治提供新的思路与策略。  

Objectives of Study：

The purpose of this study was to identify the changes of intestinal flora in patients with prediabetes and recently diagnosed type 2 diabetes, as well as the effect and possible mechanism of probiotic bifidobacterium lactobacillus M8 on blood glucose metabolism and cognitive impairment in patients with confirmed prediabetes and recently diagnosed type 2 diabetes, and to provide new ideas and strategies for the prevention and treatment of type 2 diabetes and cognitive impairment

纳入标准：

1.年龄45-80岁。 2.糖尿病前期或近期诊断的2型糖尿病患者。 糖尿病前期的诊断符合美国糖尿病协会的指南即：（i）空腹血糖水平在5.6 mmol/l至6.9 mmol/l之间，或；（ii）餐后后两小时（口服葡萄糖耐量试验）血葡萄糖水平在7.8 mmol/l至11 mmol/l之间，或；（iii）糖化血红蛋白在5.7%至6.4%之间。2型糖尿病的诊断标准基于美国糖尿病协会指南。 近期诊断的2型糖尿病患者要求在过去12个月内被内分泌医生诊断为2型糖尿病的，2型糖尿病的诊断符合以下情况之一：（i）空腹血糖水平>=7.0 mmol/l；（ii）挑战后2小时（口服葡萄糖耐量试验）血糖水平>=11.1 mmol/l或；（iii）HbA1c>=6.5%。 3.内有记忆力障碍（6个月内）主诉 4.患者意识清楚，能够自己或者在家人的帮助下配合完成检查操作、问卷调查、病史采集等。 5.同意参与本研究，签署知情同意书。

Inclusion criteria

1. Age 45-80 years old. 2. Patients with prediabetes or recently diagnosed type 2 diabetes. The diagnosis of prediabetes meets the American Diabetes Association guidelines as: (i) fasting blood glucose levels between 5.6 mmol/L and 6.9 mmol/L, or; (ii) blood glucose level between 7.8 mmol/L and 11 mmol/L two hours after a meal (oral glucose tolerance test), or; (iii) glycosylated hemoglobin between 5.7% and 6.4%. Diagnostic criteria for type 2 diabetes are based on American Diabetes Association guidelines. Patients with recently diagnosed type 2 diabetes who are required to have been diagnosed with type 2 diabetes by an endocrinologist within the past 12 months should be diagnosed with one of the following: (i) fasting blood glucose level >=7.0 mmol/l; (ii) blood glucose level 2 hours after challenge (oral glucose tolerance test) >=11.1 mmol/l or; (iii)HbA1c>=6.5%. 3. Complaints of memory impairment (within 6 months). 4. The patient is conscious and able to complete the examination operation, questionnaire survey, medical history collection, etc. by himself or with the help of his family. 5. Agree to participate in this study and sign the informed consent form.

排除标准：

1. 1型糖尿病 2. 诊断超过12个月的2型糖尿病。 3.服用二甲双胍以外的抗肥胖药物或降血糖药物（即磺酰脲类、α-葡萄糖苷酶抑制剂、噻唑烷二酮类和胰高血糖素样肽-1类似物）。 4. 阿尔茨海默病或其他痴呆症（MMSE用于排除痴呆症患者：对于文盲，MMSE评分低于17；对于接受过小学教育的参与者，MMSE小于20；对于初中及以上学历的参与者，MMSE小于24）、严重精神或神经系统疾病（如重度抑郁症、精神分裂症、帕金森病、亨廷顿氏病）、酗酒或滥用药物，或有头部创伤史或影响脑功能的系统性疾病（如系统性红斑狼疮、充血性心力衰竭）的参与者。 5.伴随的胃肠道疾病（即肠易激综合征、炎症性肠病和乳糜泻） 6.最近（在过去4周内）使用抗生素和膳食补充剂（鱼油、益生菌、益生元、多种维生素、矿物质、营养保健品和草药制剂） 7. 怀孕、哺乳或计划怀孕 8. 活动性感染或与其他任何严重疾病的临床证据。 9.对受试样品过敏或出现明显消化道症状如不能耐受者； 10.随访期间饮食习惯不规律，有较大变动的患者。

Exclusion criteria：

1. Type 1 diabetes 2. Diagnosed with type 2 diabetes mellitus more than 12 months ago. 3. Taking anti-obesity drugs or hypoglycemic drugs other than metformin (i.e., sulfonylureas, α-glucosidase inhibitors, thiazolidinediones, and glucagon-like peptide-1 analogues). 4. Alzheimer's disease or other dementia (MMSE is used to exclude patients with dementia: for illiterate, MMSE score is less than 17; For participants with primary education, the MMSE was less than 20; MMSE less than 24 for participants with junior high school education or above), severe psychiatric or neurological illness (e.g., major depressive disorder, schizophrenia, Parkinson's disease, Huntington's disease), alcohol or substance abuse, or participants with a history of head trauma or systemic disease affecting brain function (e.g., systemic lupus erythematosus, congestive heart failure). 5. Concomitant gastrointestinal disorders (i.e., irritable bowel syndrome, inflammatory bowel disease, and celiac disease) 6. Recent (within the past 4 weeks) use of antibiotics and dietary supplements (fish oil, probiotics, prebiotics, multivitamins, minerals, nutraceuticals, and herbal preparations) 7. Pregnant, breastfeeding, or planning to become pregnant 8. Active infection or clinical evidence of any other serious illness. 9. Those who are allergic to the test sample or have obvious digestive symptoms, such as intolerance; 10. Patients with irregular eating habits and large changes during the follow-up period.
