研究目的：

本研究旨在构建并探索适用于老年慢性阻塞性肺疾病（COPD）患者的数字化肺康复方案的设计与应用效果，具体目标包括以下两个方面： 1.调查与分析老年COPD患者的叙事闭锁水平及其影响因素 通过定量和定性研究方法，评估老年COPD患者的叙事闭锁水平，并探讨其主要影响因素，深入了解患者叙事闭锁的原因与内在机制，为进一步干预提供科学依据。 2.开发结合肺康复的数字化叙事护理干预措施并验证其有效性 在传统肺康复训练的基础上，融入数字化叙事护理干预手段，构建个性化的综合护理方案，旨在提升患者的叙事开放水平、改善心理健康和肺功能状态，从而提高患者的生活质量与自我管理能力。  

Objectives of Study：

This study aims to construct and explore the design and application effects of a digital pulmonary rehabilitation program for elderly patients with chronic obstructive pulmonary disease (COPD). The specific objectives include the following two aspects: 1. To investigate and analyze the level of narrative atresia and its influencing factors in elderly patients with COPD To evaluate the level of narrative atresia in elderly patients with chronic obstructive pulmonary disease (COPD) through quantitative and qualitative research methods, and to explore its main influencing factors, so as to deeply understand the causes and internal mechanisms of patients' narrative atresia and provide scientific basis for further intervention. 2. To develop and validate a digital narrative nursing intervention for pulmonary rehabilitation On the basis of traditional pulmonary rehabilitation training, digital narrative nursing intervention was integrated to construct a personalized comprehensive nursing plan, aiming to enhance the narrative openness level of patients, improve mental health and lung function status, so as to improve the quality of life and self-management ability of patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经市级医疗机构明确诊断为COPD，处于稳定期，肺功能检查为FEV1/FVC<70％和FEV1%<80％； 2.年龄≥65岁； 3.无沟通障碍，能正确应答,可独立或通过研究者帮助填写问卷； 4.小学及以上文化水平，阅读理解能力正常； 5.无精神、认知障碍，无下肢活动障碍； 6.知情同意。

Inclusion criteria

1. COPD was diagnosed by a municipal medical institution and was in a stable stage, and the pulmonary function test showed FEV1/FVC< 70% and FEV1%< 80%; 2. Age >=65 years old; 3. No communication barrier, can answer correctly, can fill out the questionnaire independently or with the help of researchers; 4. Primary school or above education level, normal reading comprehension ability; 5. No mental, cognitive impairment, and lower limb movement disorder; 6. Informed consent.

排除标准：

1.并发肺源性心脏病、呼吸衰竭；
2.伴有严重的心、肝、肾疾病；
3.近一年有心肌梗死、心绞痛发作；
4. 高血压患者高于正常范围，经药物无法控制者；
5.体质过度衰弱，不能耐受肌肉力量测试；
6.有运动后晕厥病史。

Exclusion criteria：

1. Complicated with pulmonary heart disease and respiratory failure; 2. Accompanied by serious heart, liver and kidney diseases; 3. Myocardial infarction and angina pectoris in the past year; 4. Hypertension patients are higher than the normal range and cannot be controlled by drugs; 5. Excessive physical weakness, can not tolerate muscle strength test; 6. History of fainting after exercise.
