研究目的：

主要研究目的：分别在病程>6年以及病程≤6年的2型糖尿病患者中，评估低热量代餐饮食和低热量正常食物饮食干预对减重及缓解2型糖尿病的有效性。 次要研究目的：探索低热量饮食干预缓解2型糖尿病效果的个体差异及生物学机制。  

Objectives of Study：

Primary Research Objective: To determine the effect of low-calorie meal replacement diet and low-calorie real food-based diet on weight loss and remission of type 2 diabetes in people with long diabetes duration (>6 years) and short diabetes duration (<=6 years), respectively. Secondary Research Objective: To investigate individual variations in and biological mechanisms underlying remission of type 2 diabetes following low-calorie diet intervention.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄在18-60岁； 2.符合2型糖尿病的诊断； 3.糖化血红蛋白值（HbA1c）在6.5%-12.0%之间，如在6.0%-6.4%，应正在接受降糖药物治疗； 4.BMI在24-45kg/m2； 5.近2年内无离开居住地的打算； 6.执行知情同意，并自愿参与。

Inclusion criteria

1.Age between 18-60 years; 2.Physician-diagnosed type 2 diabetes; 3.Hemoglobin A1c (HbA1c) levels between 6.5%-12.0%; if between 6.0%-6.4%, should be on hypoglycemic medication; 4.Body Mass Index (BMI) between 24-45 kg/m^2; 5.No plan of leaving the place of residence within the next 2 years; 6.Execution of written informed consent and voluntary participation.

排除标准：

1.1型糖尿病患者或目前正在使用胰岛素的2型糖尿病患者或HbA1c在12.0%以上； 2.开始前6个月发生过心血管事件； 3.目前使用抗肥胖药物治疗包括GLP-1受体激动剂，有饮食失调或有节食行为，或过去6个月内体重减轻超过5公斤； 4.慢性肾病3b期或以上（肾小球滤过率eGFR ＜30mL/min/1.73m2）； 5.任何导致液体过载的情况，如心力衰竭和肝硬化； 6.先前诊断的精神障碍（如精神分裂症、创伤后应激障碍、强迫症），未受控制的抑郁或未经控制的癫痫病； 7.严重的关节炎或活动性痛风； 8.活动性胆结石疾病或已知的无症状胆结石； 9.正在参加另一项临床研究； 10.正在怀孕、哺乳期或计划怀孕； 11.有已知的癌症； 12.有药物滥用或酗酒情况； 13.空腹C-肽＜1 ng/mL（331pmol/L）。

Exclusion criteria：

1. Patients with type 1 diabetes or patients with type 2 diabetes currently taking insulin or with an HbA1c level above 12.0%; 2. Having a cardiovascular event within the previous 6 months; 3. Currently taking weight loss medications including GLP-1 receptor agonists, having eating disorders or dieting behaviors, or weight loss exceeding 5 kilograms in the past 6 months; 4. Chronic kidney disease stage 3b or higher (eGFR < 30 mL/min/1.73 m^2); 5. Any condition causing fluid overload, such as heart failure and cirrhosis; 6. Previously diagnosed mental disorders (e.g., schizophrenia, post-traumatic stress disorder, obsessive-compulsive disorder), uncontrolled depression, or uncontrolled epilepsy; 7. Severe arthritis or active gout; 8. Active gallbladder disease or known asymptomatic gallstones; 9. Participating in another clinical study; 10. Currently pregnant, breastfeeding, or planning to become pregnant; 11. Known cancer; 12. Substance abuse or excessive alcohol use; 13. Fasting C-peptide < 1 ng/mL (331 pmol/L).

本项目采用分层区组随机化方法纳入短病程（≤6年）患者153人和长病程（>6年）患者198人，共计351名2型糖尿病患者。由专业统计人员通过计算机随机数字表法对每个招募点的研究对象进行随机分组，分层因素为肥胖程度（24-27.9kg/m2、28-32.4kg/m2和32.5-45kg/m2），区组大小为3或6，按照 1:1:1 入组。随机分配入组将由与本试验执行数据管理及统计分析无关的统计人员完成。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Using a stratified block randomization approach, this study plans to enroll 351 individuals with type 2 diabetes in total, including 153 participants with a short diabetes duration (<=6 years) and 198 with a long diabetes duration (>6 years). A statistician not involved in the management and analysis of study data will use a computer-generated random number table for each recruitment site. The stratification factor is severity of obesity (24-27.9kg/m2, 28-32.4kg/m2, and 32.5-45kg/m2). The block sizes used are 3 and 6. Enrolled participants will be randomly allocated to each of the three arms in a 1:1:1 ratio.
