研究目的：

痔疮手术是常见的外科手术之一，麻醉效果对于手术顺利开展、安全性、术后镇痛及快速康复具有重要影响。痔疮手术患者常采用折刀状俯卧位，过多使用静脉麻醉药对非插管麻醉患者易出现呼吸抑制、循环波动等并发症，增加心血管疾病及术后认知功能障碍的发生率，影响手术疗效及转归。因此痔疮手术患者选择超声引导下骶管阻滞、罗哌卡因联合适当剂量氢吗啡酮可有效增加麻醉的效果、减少麻醉并发症、减少术后疼痛及快速康复，利于预后，减少住院时间及费用，增加社会效应。  

Objectives of Study：

Hemorrhoids surgery is one of the common surgical procedures, and the anesthetic effect has an important impact on the smooth operation, safety, postoperative analgesia and rapid recovery. Patients with hemorrhoids often use a knife-shaped prone position, and excessive use of intravenous anesthetics is prone to complications such as respiratory depression and circulatory fluctuations in non-intubated anesthesia patients, which increases the incidence of cardiovascular diseases and postoperative cognitive dysfunction, and affects the efficacy and outcome of surgery. Therefore, ultrasound-guided sacral block, ropivacaine combined with appropriate dose of hydromorphone can effectively increase the effect of anesthesia, reduce anesthetic complications, reduce postoperative pain and rapid recovery for patients undergoing hemorrhoidal surgery, which is conducive to prognosis, reduces hospital stay and cost, and increases social effects.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 患者 ASA Ⅰ-Ⅱ级，年龄 l8～60 岁； 2. BMI 正常范围（18.5-23.9kg/m^2 ）； 3. 手术指征明确且无手术及麻醉禁忌证； 4. 手术方式为俯卧折刀位痔疮切除术； 5. 术前病情相对稳定，精神意识正常患者自愿参与本研究活动。

Inclusion criteria

1. Patient ASA I.-II., age l8~60 years old; 2. BMI normal range (18.5-23.9kg/m^2); 3. The indication for surgery is clear and there are no contraindications to surgery and anesthesia; 4. The surgical method is prone folding knife hemorrhoidectomy; 5. Patients with relatively stable preoperative condition and normal mental consciousness voluntarily participate in this research activity.

排除标准：

1. 合并心血管、肝、肾、脑、肺和造血系统等严重原发性疾病者； 2. 既往有精神疾病及酒精、药物依赖史者； 3. 其他器质性病变所致焦虑者； 4. 妊娠及哺乳期女性； 5. 既往有麻醉药过敏史者； 6. 二次手术者； 7. 有椎管内麻醉禁忌症或并发症及吗啡禁忌症等； 8. 低血压、心动过缓（中度以上 HR 小于 50 次/min）及窦性停搏。 5.剔除标准 9. 暂停手术治疗方案者； 10. 中途出院或死亡者； 11. 其他原因退出研究者； 12. 术中改变手术方式的患者。

Exclusion criteria：

1. Patients with serious primary diseases such as cardiovascular, liver, kidney, brain, lung and hematopoietic system; 2. Those with a history of mental illness and alcohol and drug dependence; 3. Patients with anxiety caused by other organic lesions; 4. Pregnant and lactating females; 5. Those who have a history of allergy to anesthetics in the past; 6. Second surgery; 7. Neuraxial anesthesia contraindications or complications and morphine contraindications; 8. Hypotension, bradycardia (more than moderate HR less than 50 beats/min) and sinus arrest. 5. Rejection criteria 9. Those who have suspended the surgical treatment plan; 10. Those who are discharged from the hospital or die in the middle of the hospital; 11. Withdrawal from the investigator for other reasons; 12. Patients who change the surgical method during surgery.
