研究目的：

1. 主要目的：对比单节段颈前路手术中显微镜辅助下小切口和传统切口临床疗效。 2. 次要目的：对比单节段颈前路手术中显微镜辅助下小切口和传统切口的术后并发症发生率、切口参数差异及术后患者满意度。  

Objectives of Study：

1.Primary Objective: To compare the clinical efficacy of microscope-assisted small incision versus traditional incision in single-level anterior cervical surgery. 2.Secondary Objective: To compare the postoperative complication rates, differences in incision parameters, and patient satisfaction between microscope-assisted small incision and traditional incision in single-level anterior cervical surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1） 单节段颈椎间盘疾病引起的神经根病或脊髓病患者，磁共振成像结果支持诊断，经过规范的保守治疗后无效或症状进行性加重，影响日常生活和工作，需行前路减压手术者;（2） 年龄超过 18 岁且手术节段均位于C3~C7之间 的患者;（3） 具有详细术后前后位和侧位 X 线片和临床资料的患者;（4）在心理和生理上能够完全遵守本协议书，并配合预定的治疗计划以及填写表格，并配合随访至少一年；（5）在患者签署知情同意之后意味着患者已经知晓并同意本研究的相关方面实施开展。

Inclusion criteria

(1) Patients with radiculopathy or myelopathy caused by single-level cervical disc disease, with magnetic resonance imaging results supporting the diagnosis, who have failed to respond to standard conservative treatment or have progressive symptoms that affect daily life and work, and require anterior decompression surgery; (2) Patients over 18 years old with surgical levels located between C3 and C7; (3) Patients with detailed preoperative and postoperative anterior-posterior and lateral X-ray films and clinical data; (4) Patients who are mentally and physically capable of fully complying with this agreement, following the prescribed treatment plan, completing forms, and participating in follow-up for at least one year; (5) Patient's signing of the informed consent implies that they have been informed of and agree to the relevant aspects of this study being implemented and conducted.

排除标准：

（1） 术前吞咽困难患者;（2）患有躁狂症、严重抑郁症等心理疾病或有创伤性脑损伤、脑卒中等中枢神经系统疾病史的患者;（3） 既往有颈椎手术史、邻近节段骨性融合、有食管疾病、翻修手术和颈部手术史的患者;（4） 颈部皮肤局部感染、破溃，椎骨病理性骨折、脊柱畸形、后纵韧带 （OPLL） 骨化、类风湿性关节炎、强直性脊柱炎、发育性狭窄和局部或全身感染的患者；（5）全身情况条件差，重要脏器患有严重疾病、不能耐受手术；（6）妊娠或研究期间有妊娠计划者；（7）正在参加其他可能对本研究结果造成影响的研究。

Exclusion criteria：

(1) Patients with preoperative dysphagia; (2) Patients with psychological disorders such as mania, severe depression, or a history of central nervous system diseases including traumatic brain injury, stroke, etc.; (3) Patients with a history of previous cervical spine surgery, adjacent segment bony fusion, esophageal diseases, revision surgeries, or neck surgeries; (4) Patients with local infection or ulceration of the neck skin, pathological fractures of the vertebrae, spinal deformities, ossification of the posterior longitudinal ligament (OPLL), rheumatoid arthritis, ankylosing spondylitis, developmental stenosis, and local or systemic infections; (5) Patients with poor overall health status, severe diseases of vital organs, or intolerance to surgery; (6) Pregnant women or those planning pregnancy during the study period; (7) Patients currently participating in other studies that may affect the results of this study.

盲法：

单盲（针对研究参与者设盲，使研究参与者不知道其所接受的手术使用小切口还是传统切口。由于本研究是在比较两种手术切口，术者需要进行手术操作，所以无法设盲）。（由于对照组显微镜辅助下传统切口约 5cm，本研究干预的实验组显微镜辅助下小切口约 3cm，目前手术切口选择基本均为传统切口，传统切口与小切口外观区别差异并不是非常明显，因此接受手术治疗的患者术后不知道接受的是小切口还是传统切口）。
