研究目的：

1.1探索肌少症在TIPS病人特异性的SMI截断值，并评估TIPS术前肌少症与术后1年内发生HE的相关性及其程度 1.2基于术前肌少症以及其他变量建立术后1年内发生HE的预测模型，并验证模型的预测价值  

Objectives of Study：

1.1 Explore the specific SMI cut-off values for sarcopenia in patients undergoing TIPS and assess the association and extent of pre-TIPS sarcopenia with hepatic encephalopathy (HE) occurrence within one year post-procedure. 1.2 Develop a predictive model for the occurrence of HE within one year post-TIPS based on preoperative sarcopenia and other variables, and validate the predictive value of the model.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.各种病因的肝硬化通过临床资料结合影像学或肝活检进行确诊。 2.肝硬化的患者因为合理的适应症施行TIPS手术，包括静脉曲张破裂出血、难治性腹水等。 3.在就诊医院术前有腹部CT检查影像资料以评估肌少症。

Inclusion criteria

1. Cirrhosis from various etiologies is confirmed through clinical data combined with imaging or liver biopsy. 2. Cirrhotic patients undergo TIPS procedure based on appropriate indications, such as variceal bleeding and refractory ascites. 3.Patients have preoperative abdominal CT imaging available at the treating hospital to assess sarcopenia.

排除标准：

1.年龄小于18岁。 2.肝硬化诊断不明确，或因其他原因行TIPS手术如布加综合征、特发性门脉高压症等。 3.既往已行TIPS术，本次因支架堵塞再次行TIPS术。 4.患者腹部CT成像效果不佳，或不包含腰3层面，或缺少CT动态影像资料（如只有胶片），或术前CT距离手术超过3个月。 5.患者合并神经或精神类疾病、或口服抗焦虑、抑郁等精神类药物，干扰术后HE的评估。 6.患者合并进展性肝癌（超过Milan标准）、其他系统恶性肿瘤、或严重慢性疾病（如慢性肾脏病4期或以上）影响病人的预后。 7.患者临床资料不全或缺失； 8.患者在3个月内失访； 9.前瞻性队列病人拒绝签署知情同意，或中途要求退出。

Exclusion criteria：

1. Age under 18 years. 2. Uncertain cirrhosis diagnosis or undergoing TIPS for other reasons, such as Budd-Chiari syndrome or idiopathic portal hypertension. 3. Previously received TIPS, with current procedure performed due to stent occlusion. 4. Poor quality abdominal CT imaging, lack of third lumbar vertebra level imaging, absence of dynamic CT images (e.g., only film available), or preoperative CT performed more than 3 months before surgery. 5. Presence of neurological or psychiatric disorders, or current use of anti-anxiety or antidepressant medications interfering with postoperative HE assessment. 6. Advanced liver cancer exceeding Milan criteria, other systemic malignancies, or severe chronic diseases (e.g., stage 4 or higher chronic kidney disease) impacting patient prognosis. 7. Incomplete or missing clinical data. 8. Loss to follow-up within 3 months. 9. Prospective cohort patients who declined to sign informed consent or requested withdrawal during the study.
