研究目的：

心力衰竭是目 21 世纪最严峻的心血管疾病之一，随着人口的老龄化及高血压、糖尿病、冠心病人数攀升，它的流行程度预计到 2030 年增长 25%。尽管以“新四联”为代表的抗心衰药物在临床实践中已经显示出卓越的心血管保护作用，仍有大量的心衰患者并不能从中获益，尤其在高龄老人中，由于衰弱、肌少症等老年综合症的存在，目前心衰药物使用并不理想，因此迫切需要针对高龄心衰患者开展进一步研究。本项目拟基于 2018-2023 四川大学华西医院高龄心衰住院患者，通过前瞻性与回顾性队列研究，并综合运用代谢组学等多种研究手段发现高龄心衰相关生物标志物，探究心衰患者发病、预后预测因素，优化高龄心衰患者治疗方案，进一步推进高龄心衰老人的精准医疗方案开发。  

Objectives of Study：

Heart failure is one of the most serious cardiovascular diseases of the 21st century, and its prevalence is expected to increase by 25% by 2030 as the population ages and the number of people with hypertension, diabetes, and coronary heart disease rises. Although the anti-heart failure drugs represented by the ‘new quadruple combination’ have shown excellent cardioprotective effects in clinical practice, a large number of patients with heart failure do not benefit from them, especially in the elderly, where the use of heart failure drugs is not ideal due to the presence of senile syndromes such as debilitation and sarcopenia, so further research is urgently needed for elderly patients with heart failure. heart failure patients for further research. This project aims to identify biomarkers related to heart failure in the elderly through prospective and retrospective cohort studies based on elderly heart failure inpatients at West China Hospital of Sichuan University from 2018 to 2023, and comprehensively use metabolomics and other research tools to explore the predictive factors of the onset and prognosis of heart failure patients, optimise the treatment plan for elderly heart failure patients, and further promote the development of precision medical solutions for elderly heart failure patients.

纳入标准：

1.依据2024年中国心力衰竭指南，符合射血分数降低的心衰诊断患者； 1.1.存在心衰的症状或体征； 1.2.存在心脏结构或功能的异常； 1.3.LVEF < 40%。 2.年龄大于等于80岁； 3.于四川大学华西医院至少有过一次住院的患者。

Inclusion criteria

1. Patients who met the diagnosis of heart failure with reduced ejection fraction according to the 2024 Chinese Heart Failure Guidelines; 1.1. Presence of symptoms or signs of heart failure; 1.2. Presence of cardiac structural or functional abnormalities; 1.3. LVEF < 40%. 2. Age >= 80 years old; 3. Patients with at least one hospitalization in West China Hospital of Sichuan University.

排除标准：

1.被诊断为急性心肌炎，或肥厚型心肌病，或限制性心肌病，或缩窄性心肌病。 2.心脏移植受试者或入院行心脏移植、左心室辅助装置手术。 3.除植入式心律转复除颤器和/或心脏再同步化治疗外，4周内预计会行心脏外科手术或心脏介入手术。 4.目前已知无法遵守医嘱或后续指导建议。 5.使用中国尚未批准的药物及设备进行治疗。 6.伴随有严重的呼吸系统疾病、严重的瓣膜性疾病、急性冠脉综合征、或者中风，且门诊或入院主要诊断并非心力衰竭。

Exclusion criteria：

1. A diagnosis of acute myocarditis, or hypertrophic cardiomyopathy, or restrictive cardiomyopathy, or constrictive cardiomyopathy. 2. Heart transplant subjects or admission for heart transplantation and left ventricular assist device surgery. 3. Cardiac surgery or cardiac intervention in addition to implantable cardioverter-defibrillator and/or cardiac resynchronization therapy is expected within 4 weeks. 4. Known inability to adhere to medical orders or follow-up recommendations. 5. Treatment with drugs and devices not yet approved in China. 6. Concomitant severe respiratory disease, severe valvular disease, acute coronary syndrome, or stroke with a non-heart failure as the primary outpatient or admission diagnosis.
