研究目的：

1.评价延续性护理模式下患者的近期疗效(90 天内)和远期疗效(180 天)； 2.评价延续性护理模式下患者的依从性； 3.评价延续性护理模式下患者的满意度； 4.评价延续性护理模式的社会经济效益。  

Objectives of Study：

1. To evaluate the short-term effect (within 90 days) and long-term effect (180 days) of patients under continuous care model; 2. To evaluate patient compliance in continuous care; 3. To evaluate the satisfaction of patients under continuous care model; 4. To evaluate the socio-economic benefits of continuous care.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.患者年龄 18-60 岁； 2.根据国际尿控协会标准诊断为压力性尿失禁； 3.在盆底医学中心接受规范盆底康复住院治疗，且治疗结束后需出院继续康复的患者； 4.拥有智能手机，能接触网络； 5.能提供知情同意的患者。

Inclusion criteria

1. The patient is 18-60 years old; 2. The patient was diagnosed as stress urinary incontinence according to the standards of the International Association for Urinary Control; 3. Patients who receive standardized pelvic floor rehabilitation hospitalization in the pelvic floor medical center and need to be discharged to continue rehabilitation after treatment; 4. Have a smart phone and be able to access the network; 5. Patients who can provide informed consent.

排除标准：

1.不符合入选标准者的其他类型尿失禁、如混合型、复杂性尿失禁、尿道憩室、 生殖道瘘等； 2.患者随访依从性差； 3.患者存在影响认知功能的精神系统疾病。

Exclusion criteria：

1. Other types of urinary incontinence that do not meet the inclusion criteria, such as mixed type, complex urinary incontinence, urethral diverticulum, genital fistula, etc; 2. Poor follow-up compliance of patients; 3. The patient has mental system diseases that affect cognitive function.
