研究目的：

1. 探索急性胰腺炎重症化、反复发作以及转归为慢性胰腺炎、胰腺癌的影响因素。 2. NK细胞在胰腺炎重症化过程中的作用及机制，明确NK细胞胰腺浸润与胰腺炎胰腺细胞坏死的关系，活化的NK细胞对胰腺炎细胞是攻击还是保护？ 3. 中性粒细胞在胰腺炎重症化过程中的作用及机制，明确中性粒细胞胰腺浸润与胰腺炎胰腺细胞坏死的关系，活化的中性粒细胞对胰腺炎细胞是攻击还是保护？ 4. NK细胞与中性粒细胞的交互作用是否为急性胰腺炎重症化的决定因素？（通过分析NK细胞与中性粒细胞受体-配体的表达及下游通路信号通路的活化，分析NK细胞与Treg调节性中性粒细胞的相互作用）  

Objectives of Study：

1. to explore the influencing factors of acute pancreatitis re-conditioning, recurrent episodes and regression to chronic pancreatitis and pancreatic cancer. 2. the role and mechanism of NK cells in the process of pancreatitis re-conditioning, to clarify the relationship between NK cell pancreatic infiltration and pancreatic cell necrosis in pancreatitis, and whether activated NK cells are attacking or protecting pancreatitis cells? 3. the role and mechanism of neutrophils in the process of pancreatitis remittentization, clarify the relationship between neutrophil pancreatic infiltration and pancreatic cell necrosis in pancreatitis, and whether activated neutrophils attack or protect pancreatitis cells? 4. Is the interaction between NK cells and neutrophils a determinant of acute pancreatitis remittentization? (Analyze the interaction of NK cells with Treg regulatory neutrophils by analyzing the expression of NK cells and neutrophil receptor-ligands and the activation of downstream pathway signaling pathways)

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄在18至70岁之间 2. 发病后48小时内 3. 具有AP的初步诊断;满足以下３条标准中的２条：①具有急性胰腺特征性的腹痛；②血清淀粉酶和（或）脂肪酶多正常上限３倍；③具有急性胰腺炎特征性的影像学表现。

Inclusion criteria

1. Between 18 and 70 years of age 2. Within 48 hours of onset 3. Have a preliminary diagnosis of AP; meet 2 of the following 3 criteria: 1) have abdominal pain characteristic of acute pancreas; 2) have serum amylase and/or lipase 3 times more than the upper limit of normal; 3) have imaging manifestations characteristic of acute pancreatitis.

排除标准：

1. 直接参与试验的研究人员的家庭成员 2. 已知有严重心血管、呼吸系统、肾脏或肝脏疾病病史的患者，定义为（1）II级以上心力衰竭，（2）活动性心肌缺血或（3）既往 60 天内的心血管介入治疗，（4）肝硬化史或（5）肌酐清除率为 <40 mL/min 的慢性肾脏疾病或（6）慢性阻塞性肺病、患有艾滋病等预先存在的免疫性疾病的患者 3. 妊娠胰腺炎、慢性胰腺炎病史、恶性肿瘤相关AP 4. 研究人员认为没有理由纳入，例如，预计患者在入组后48小时内死亡 5. 由下级医院转入，且发病时间已超过48小时

Exclusion criteria：

1. family members of investigators directly involved in the trial 2. patients with a known history of severe cardiovascular, respiratory, renal, or hepatic disease, defined as (1) class II or greater heart failure, (2) active myocardial ischemia or (3) previous cardiovascular intervention within 60 days, (4) a history of cirrhosis of the liver, or (5) chronic renal disease with a creatinine clearance of <40 mL/min, or (6) pre-existing diseases such as chronic obstructive pulmonary disease (COPD), HIV/AIDS, etc. Patients with pre-existing immunologic diseases such as AIDS 3. pregnancy pancreatitis, history of chronic pancreatitis, malignancy-related AP 4. patients who, in the opinion of the investigator, are not justified for inclusion, e.g., patients expected to die within 48 hours of enrollment 5. transfer from a lower level hospital where the onset of illness has exceeded 48 hours
