研究目的：

本研究旨在评估通过家庭医生干预措施对老年共病患者药物依从性及疾病管理效果的影响，从而为提高老年人生活质量、减少因药物依从性差而导致的再住院和医疗负担提供实践基础，为推进社区健康管理体系建设提供借鉴。  

Objectives of Study：

The purpose of this study is to evaluate the impact of family doctor interventions on medication adherence and disease management outcomes among elderly patients with multimorbidity. The aim is to provide a practical foundation for improving the quality of life of the elderly, reducing rehospitalization, and alleviating the healthcare burden caused by poor medication adherence, as well as offering insights for advancing the community health management system.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 患有两种及以上常见的心血管疾病（高血压、冠心病、血脂异常、糖尿病）； 2) 年龄在60周岁以上； 3) 在纳入研究之前至少3个月内至少服用过一种治疗慢性病的药物； 4）能使用手机基本功能（电话、短信等）； 5）能独立管理您正在服用的药物； 6) 意识清楚、有生活自理能力和配合能力，病情稳定； 7) 具有一定的运动能力，可以进行治疗所需的基本运动； 8）具有听说读写等基本能力，能进行有效沟通； 9）能够提供参与研究的书面知情同意书。

Inclusion criteria

1) Diagnosed with two or more common cardiovascular diseases (hypertension, coronary heart disease, dyslipidemia, diabetes); 2) Aged 60 years or above; 3) Have been taking at least one medication for chronic disease for at least three months prior to inclusion in the study; 4) Able to use basic phone functions (calls, text messages, etc.); 5) Capable of independently managing their current medications; 6) Clear consciousness, able to live independently and cooperate, with stable condition; 7) Have sufficient physical ability to perform basic therapeutic exercises; 8) Have basic reading, writing, listening, and speaking abilities for effective communication; 9) Able to provide written informed consent to participate in the study.

排除标准：

1) 最近有严重的病史（如心衰竭，过去3个月的心肌梗死，冠状动脉搭桥手术或冠状动脉再成形术，不稳定型心绞痛或不受控制的严重心律失常，严重的慢性阻塞性肺病，需要手术、放疗或化疗的癌症）； 2) 预期寿命少于12个月； 3) 被安置在疗养院或长期护理机构； 4）计划在未来6个月内离开社区； 5）有认知障碍或严重的精神问题； 6）正在参加另一项涉及慢性病管理的研究。

Exclusion criteria：

1) Recent history of serious illness (e.g., heart failure, myocardial infarction within the past 3 months, coronary artery bypass surgery or coronary revascularization, unstable angina, or uncontrolled severe arrhythmia, severe chronic obstructive pulmonary disease, cancer requiring surgery, radiotherapy, or chemotherapy); 2) Life expectancy of less than 12 months; 3) Placement in a nursing home or long-term care facility; 4) Planning to leave the community within the next 6 months; 5) Cognitive impairment or severe mental health issues; 6) Currently participating in another study related to chronic disease management.
