研究目的：

主要目的：评估单颗磨牙延期种植即刻修复与延期修复在种植体周围骨重建方面的效果差异，比较两者在骨水平变化中的临床疗效。 次要目的：评估即刻修复在种植体成功率、软组织健康（如龈乳头指数、角化龈宽度、探诊深度、出血指数等）、修复体美学效果、患者满意度及术后不适感、并发症发生率，以及在经济效益方面的表现。  

Objectives of Study：

Primary Objective: To evaluate the differences in peri-implant bone reconstruction outcomes between immediate restoration and delayed restoration of single molar delayed implants, and to compare their clinical efficacy in terms of bone level changes. Secondary Objectives: To assess the performance of immediate restoration in terms of: Implant success rate Soft tissue health (e.g., papilla index, keratinized gingiva width, probing depth, bleeding index, etc.) Aesthetic outcomes of the restoration Patient satisfaction and postoperative discomfort Incidence of complications Economic benefits

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）愈合时间：患者的牙齿已经拔除至少3个月，确保拔牙创口完全愈合并达到良好的骨愈合状态，种植位点处的骨量和骨密度足够，能够提供种植体初始稳定性。 （2）种植位点无急性炎症或感染：种植位点、邻牙的牙周以及根尖周组织没有急性病变，确保种植体植入时周围组织的健康状态。 （3）良好的软组织条件：种植位点周围的软组织健康，没有明显的炎症或病变。 患者全身健康状况良好：患者没有严重的全身性疾病或控制不良的慢性病，如未控制的糖尿病、严重的骨质疏松症等。 （4）无严重的咬合或颌骨问题：患者的咬合关系和颌骨结构适合种植手术，没有严重的咬合不正或颌骨畸形。 （5）患者口腔卫生状况良好：患者有良好的口腔卫生习惯，能够在术后保持口腔卫生。 （6）无不良的口腔习惯：严重的磨牙症、咬硬物等不良习惯。

Inclusion criteria

Healing Time: The tooth has been extracted for at least 3 months, ensuring complete healing of the extraction site with good bone regeneration. The bone volume and density at the implant site must be sufficient to provide initial stability for the implant. No Acute Inflammation or Infection at the Implant Site: There are no acute pathological conditions at the implant site, in the adjacent teeth, or in the periodontal and periapical tissues, ensuring healthy surrounding tissues during implant placement. Good Soft Tissue Condition: The soft tissues around the implant site are healthy, with no significant inflammation or pathological changes. Good General Health: The patient has no severe systemic diseases or uncontrolled chronic conditions, such as uncontrolled diabetes or severe osteoporosis. No Severe Occlusal or Jaw Issues: The patient’s occlusion and jaw structure are suitable for implant surgery, with no severe malocclusion or jaw deformities. Good Oral Hygiene: The patient maintains good oral hygiene habits and is capable of maintaining oral hygiene postoperatively. No Harmful Oral Habits: The patient does not have severe bruxism, habits of biting hard objects, or other detrimental habits.

排除标准：

（1）存在严重的心脑血管疾病和癫痫等神经系统疾病史者； （2）存在全身性系统性疾病比如糖尿病、血液病且未控制者； （3）长期服用双膦酸盐类药物者； （4）重度吸烟（＞20 支/天）或嗜酒者； （5）存在由于任何疾病导致的张口受限者； （6）患牙或邻近组织存在急性炎症者； （7）妊娠期和哺乳期妇女。

Exclusion criteria：

History of Severe Cardiovascular, Cerebrovascular, or Neurological Diseases: Includes conditions such as epilepsy or other severe neurological disorders. Uncontrolled Systemic Diseases: Patients with uncontrolled systemic conditions, such as diabetes or blood disorders. Long-term Use of Bisphosphonates: Individuals undergoing prolonged bisphosphonate therapy. Heavy Smoking or Alcohol Abuse: Patients who smoke heavily (more than 20 cigarettes/day) or have a habit of excessive alcohol consumption. Limited Mouth Opening Due to Any Condition: Patients with restricted mouth opening caused by any underlying disease. Acute Inflammation in the Affected Tooth or Adjacent Tissues: Presence of acute inflammation in the tooth or its surrounding tissues. Pregnant or Lactating Women: Women who are pregnant or currently breastfeeding.

Blinding：

A single-blind design is adopted. While the participants and the medical staff performing the surgery are aware of the specific treatment allocation, the outcome assessors and data analysts remain blinded to the group assignments to minimize evaluation bias. In case of emergencies during the study that require unblinding, the process will be managed by the study supervisor or an independent third party to ensure the integrity of the study and the safety of the participants.
