研究目的：

比较不同方法个体化滴定PEEP(Positive end expiratory pressure)引导下的肺保护性通气策略对围术期腹腔镜下肥胖患者肺保护及术后肺不张的影响,探讨滴定呼气末正压(PEEP)的最佳方法的选择,促进手术患者快速康复。  

Objectives of Study：

To compare the effects of pulmonary protective ventilation strategies guided by different methods of personalized titration of PEEP (Positive end expiratory pressure) on lung protection and postoperative atelectasis in obese patients under perioperative laparoscopy, and to explore the selection of the best method for titration of positive end-expiratory pressure (PEEP) to promote the rapid recovery of surgical patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

性别不限，年龄18～45岁，BMI ≥30/m2，ASA分级I～Ⅲ级。

Inclusion criteria

Gender is not limited, age 18 to 45 years old, BMI >= 30/m^2, ASA grade I to III.

排除标准：

(1) 有严重影响胸廓或肺顺应性的疾病或肺部手术史，如鸡胸、漏斗胸、胸部外伤等； (2) 有严重心、肺疾病及脑血管、肝、肾、血液系统疾病等； (3) 发生严重血流动力学波动，补液和血管活性药物难以纠正； (4) 术中无法维持氧和(SpO2<94%或PaO2<70mmHg),或者发生机械通气相关严重并发症（如气胸）。

Exclusion criteria：

(1) Have a history of diseases or lung surgery that seriously affect thoracic or lung compliance, such as chicken breast, funnel chest, chest trauma, etc; (2) Have serious heart and lung diseases, cerebrovascular, liver, kidney, and blood system diseases, etc; (3) Severe hemodynamic fluctuations occur, making it difficult to correct with fluid replacement and vasoactive drugs; (4) Inability to maintain oxygen during surgery (SpO2 < 94% or PaO2 < 70 mmHg), or severe complications associated with mechanical ventilation (e.g. pneumothorax).
