Objectives of Study: |
This study aims to comprehensively investigate perioperative neurocognitive disorders (PNDs) in elderly patients based on the 2018 consensus, employing machine learning and SHAP methodologies across epidemiological, etiological, preventive diagnostic, and technical levels: 1) to obtain high-quality epidemiological data on PNDs using consensus criteria; 2) to identify key risk factors through machine learning; 3) to develop a risk assessment tool with high predictive performance and interpretability; and 4) to explore the application of SHAP in PND risk prediction. Utilizing a prospective, multi-center design, this study will assess the incidence of early and late postoperative cognitive dysfunction and employ machine learning and SHAP to analyze key risk factors, ultimately providing evidence for individualized prevention and treatment strategies for PNDs. |

本研究拟纳入2025年1月至2025年12月择期接受非心脏大手术的60-80岁老年患者。 具体纳入标准如下: 1)年龄60-80岁; 2)择期行以下类型手术之一:腹部手术,胸科手术,骨科创伤性骨折手术或髋关节、膝关节置换术; 3)自愿参加并签署知情同意书。 |

Inclusion criteria |
This study plans to enroll elderly patients aged 60-80 years who are scheduled to undergo major non-cardiac surgery between January 2025 and December 2025. The specific inclusion criteria are as follows: 1). Age between 60 and 80 years; 2). Scheduled to undergo one of the following types of surgery: abdominal surgery, thoracic surgery, orthopedic surgery for traumatic fractures, or hip or knee replacement surgery; 3). Willing to participate and sign an informed consent form. |

排除标准: |
排除标准包括: 1)拒绝研究知情同意; 2)因视力、听力、语言等问题,无法有效交流; 3)既往有中枢神经系统疾病史(中风、颅脑外伤等); 4)既往有精神疾病史(精神分裂症、抑郁症等); 5)存在重大躯体疾病(瘫痪、截肢等); 6)近1年内有重大手术史(肿瘤根治术、心脏手术等); 7)重要脏器系统严重功能障碍; 8)近1月内规律服用抗精神病类、镇静催眠类药物; 9)术前MMSE评分低于教育程度对应的临界值(文盲≤17分,小学≤20分,初中及以上≤24分)。 |

Exclusion criteria: |
Exclusion Criteria: 1). Refusal of study informed consent; 2). Inability to communicate effectively due to issues with vision, hearing, language, etc; 3). History of central nervous system disorders (stroke, traumatic brain injury, etc.); 4). History of mental illness (schizophrenia, depression, etc.); 5). Presence of significant physical illnesses (paralysis, amputation, etc.); 6). History of major surgery within the past year (radical resection for tumors, cardiac surgery, etc.); 7). Severe dysfunction of major organ systems; 8). Regular use of antipsychotic or sedative-hypnotic medications within the past month; 9). Preoperative MMSE score below the cut-off value corresponding to education level (illiterate <=17, primary school <=20, junior high school and above <=24). |

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture: |
This study will utilize paper-based Case Record Forms (CRFs) and the RedCap electronic data capture system. Data will be initially recorded on CRFs and subsequently entered into the secure RedCap system. RedCap was chosen for its security, user-friendliness, and data validation capabilities. Data will be entered independently by two researchers and cross-checked. RedCap will also implement data quality checks. All data will be securely stored in RedCap, accessible only to authorized personnel. Data will be regularly backed up and archived according to ethical guidelines upon study completion. |
